Thank you to CE Healthlink, Melisa Marra, Michelle Soltman, and the entire leadership team for hosting such a meaningful educational event focused on “Bridging the Gap: Curative Care to Hospice Conversations.”

Lily Hospice was honored to participate in an evening dedicated to education, collaboration, and improving the way we support patients and families through important healthcare transitions.

We would also like to thank Martha T. Berry Medical Care Facility for graciously hosting and helping bring together such an engaged group of nurses, social workers, case managers, and healthcare professionals.

These conversations matter, and we are grateful to be part of a healthcare community committed to compassionate, patient-centered care.
