Dialysis keeps many people alive for years. It also asks a lot: three sessions a week in the chair, the drive, the washed-out hours afterward, a diet that shapes every meal. The calendar does not stop for illness. Thanksgiving comes, then Christmas, Hanukkah, New Year's, a birthday. Families caring for someone on hospice often dread those days: the pressure to make it special, the fear it is the last one, the relatives arriving with opinions. You do not have to cancel the holidays. Most families shrink them and bring them to wherever the person is: one favorite dish, familiar music, short visits, a video call for the ones far away, and a plan made with the hospice nurse for the medical side. A stroke arrives in a single afternoon and then asks a family to make decisions for months. Some families are asked about goals of care within the first week at the hospital. Many more come home from rehab, plateau, and only later notice the swallowing trouble, the repeated pneumonia, the weight loss, the sleeping most of the day. Most tell us afterward they wish they had called sooner. Our Veteran Advisory Committee held its first meeting on October 7. It is chaired by Navy SEAL veteran Evan Shelline, with our own Simone Luu, RN, a former Navy Hospital Corpsman, as co-chair and clinical leader. Members served in the Army, Navy, Air Force and Marine Corps.

"A neighbor told me hospice isn't in Dad's Medicare Advantage plan." We hear that every week, and it is not true. When an Advantage member elects hospice, Original Medicare Part A pays for it directly: the nurse, the aide, the hospital bed, comfort medications, the social worker and chaplain, respite for the caregiver, and bereavement support afterward. His plan stays exactly as it is for everything unrelated, his plan's doctor can stay on as his attending physician, and the plan's network does not limit which hospice you choose.
