✅ Option 1: Call our office and leave your child's name and date of birth on the nurse line.
📧 Option 2: Email your child's name and date of birth to [email protected].
🏥 Option 3: Already in the office? Let your Medical Assistant know, and we'll be happy to get you a completed copy.

☀️ Sunshine, smiles, and our favorite people! 🩷🌸

Our CHA Novi team traded scrubs for matching pink shirts and spent the day enjoying our Novi office building summer picnic! 🍉🌭 From great food to lots of laughs, it was the perfect way to celebrate summer and spend time together outside the office.

🍨 🍦 I scream. You scream. We all scream for ICE CREAM.. especially when National Ice Cream Day is this Sunday! 🍦

Today our team took a few minutes to celebrate National Ice Cream Day with some sweet treats and even sweeter company. We love caring for our patients, but we also love taking time to celebrate the little moments together.
