Communicare Michigan - CMI is dedicated to providing post trauma, brain injury survivors and individuals with other neurologic impairments the skills and supports necessary to attain their maximum level of functioning and quality of life in a least restrictive community environment, and to make informed choices and decisions about their lives. Communicare Michigan provides Neurocognitive and Neuro

behavioral services to individuals with cognitive impairment through a functional, community integrated and multidisciplinary approach. Clients are provided unique opportunities to receive clinical, vocational and life skills services within community based Programs and Clubhouses through out Oakland and Macomb Counties in Michigan. Vocational services are offered through various work programs which simulate real life positions, responsibilities and performance. Communicare Michigan's approach produces functional and naturalized learning in an environment generalizable for goal acquisition. Troy Program:

Last night we gathered for our Summer Bloom BBQ at CommuniCare in Grand Rapids. The event was co-hosted by CommuniCare, Brightstar Care, and Mitten Care Case Management.

It was wonderful to collaborate and catch up with so many dedicated healthcare and case management professionals in West MI. We love working together to support the clients and patients we serve every day!
