Community Health Connection Serving patients at our 4 locations. Kendall-Whittier, Ellen Ochoa, Rosa Parks School, and Youth Services of Tulsa.

Community Health Connection, Inc is a 501(c) 3 Federally Qualified Health Center in Tulsa, Oklahoma. The Mission of Community Health Connection is to provide affordable, accessible quality healthcare across all life cycles regardless of ability to pay that respects the dignity and culture of our community now and in the future. Community Health Connection is a health clinic, but also has dental services, behavioral health services and a pharmacy. We accept most private insurance, Medicaid and Medicare. For uninsured patients, we base what we charge on a sliding scale which is based on your income and household size.

All CHC health centers are closed today (October 12) to honor Indigenous People's Day and reflect on and celebrate the rich histories, cultures, and resilience of the Native communities who have shaped Green Country and beyond. We are privileged to care for Oklahoma's Indigenous population and remain committed to supporting health equity across all cultures.

We will reopen at normal hours on Tuesday, October 13.

Important changes are coming to SoonerCare (Medicaid) because of a new federal law.

Starting January 1, some SoonerCare members must meet new work requirements to keep their coverage. If the Oklahoma Health Care Authority cannot verify that you meet the requirement or qualify for an exception, they may ask you for more information. It is important to keep your information up to date and watch closely for notices.

Did you recently get a yellow letter in the mail? 📬

If so, you may need to meet new work requirements to keep your coverage beginning in January.

Here’s what to do now:
✅Update your contact information in your MySoonerCare portal
✅Check your mail, email and MySoonerCare portal often
✅Get ready to report work, school or volunteer activities

OHCA will contact members when it’s time to take action, so keeping your
information up to date is really important to make sure you don’t lose coverage.

spread the word! If you need help finding work, education or volunteer opportunities, check out the link above for resources. As always, you can contact one of our Community Health Workers with questions or renewal assistance.

It’s so easy to celebrate the people we care for, the partners we work alongside, the neighborhoods we serve, and the community that continues to show up for one another day in and day out. We’re so grateful to have called the 918 home for over 20 years now, and what an honor it is to continue to grow with this city as we continue to make progress on our FIFTH location here in town.
