Letter from Tyler Poe:
To My Patients,
After much reflection, I have made the difficult decision to leave Taylorville Family Medicine. My last day with the practice will be August 5, 2026. Writing this letter is one of the hardest things I have had to do. While I am excited for the opportunities that lie ahead, leaving the privilege of caring for each of you is incredibly
difficult. When I accepted my first position as a Nurse Practitioner, I hoped I would be able to make a difference in the lives of my patients. What I never expected was just how much
my patients would change my life.
Thank you for trusting me with your care. Thank you for allowing me to celebrate your victories, support you through difficult diagnoses, listen to your concerns, laugh with you,
and sometimes simply sit with you during life's hardest moments. Being invited into those moments is a privilege I have never taken for granted. Many of you came to know me as a brand-new Nurse Practitioner. You extended me grace, patience, and trust while I grew into this profession. Together we celebrated healthier blood pressures, improved A1Cs, recovered illnesses, new babies, graduations, retirements, and countless other moments that made each day meaningful.
We also faced heartbreak, uncertainty, and loss together. Through every visit, you reminded me why I chose healthcare. You have taught me lessons that no classroom ever could. You taught me resilience in the face of illness, courage during uncertainty, humor when life felt overwhelming, and the importance of treating every person with dignity and compassion. I hope that in some
small way I was able to give back a fraction of what each of you gave to me.
As your provider, I always tried to remember that I wasn't simply treating numbers on a chart... I was caring for mothers and fathers, grandparents, children, spouses, friends, veterans, teachers, laborers, and neighbors. Every person who walked through the door brought a story, and it has been one of the greatest honors of my life to be entrusted with a small part of those stories.
Although I will no longer be practicing at Taylorville Family Medicine, I have every confidence that the team will continue to provide you with compassionate, high-quality care. I encourage you to continue prioritizing your health, keeping your appointments,
asking questions, and advocating for yourself. Your health matters, and you deserve providers who will listen and partner with you in your care.
As I begin this next chapter, I will carry the memories of our time together with me. I will remember your smiles, your determination, your stories, and the trust you placed in me. Those memories will continue to remind me why I chose this profession and will inspire
the kind of provider I strive to be every day.
Thank you for allowing me the extraordinary privilege of being your Nurse Practitioner. Caring for you has been far more than my job, it has been one of the greatest honors of my life.
I wish you and your families continued health, peace, and happiness in the years ahead.
