📚 Back-to-School Reminder: Updated School Medication Forms Required

As students head back to school over the coming days and weeks, we want to remind families about an important update from the Alabama Department of Education.

✅ The school medication authorization form has been updated for the new school year.

If your child needs to receive or self-administer medication at school, a completed medication form must be on file with the school. These forms are required to be updated every school year and must be completed for each medication your child uses at school.

To help ensure your child is ready for the first day, we encourage families to submit medication form requests to our office as soon as possible. As the start of school approaches, turnaround times may increase due to the higher volume of requests.

📌 Important Reminders:
• A new medication form is required each school year.
• A separate form is needed for each medication.
• Forms must be on file with your child's school for medications that are administered or self-carried at school.
• The physician prescribing the medication is responsible for completing the medication authorization form. If your child receives a medication from a specialist, please contact that specialist's office to have the form completed.
• The updated medication form can be found on your school's website.

Planning ahead helps ensure your child has uninterrupted access to any medications they may need during the school day. Thank you for helping us prepare for a safe and successful school year!

Dr. Turley recently completed his pediatric residency training with the UAB Heersink School of Medicine, where he distinguished himself through his dedication to providing exceptional care for children and families. During his residency, Dr. Turley was honored with the 2026 Carolyn Ashworth, M.D. Pediatric Primary Care Award, which recognizes excellence in delivering compassionate, high-quality pediatric primary care.
