Medical Records - update- due to some technical difficulties we are having to change our fax number and phone number for medical record request. We are aware that some records have not been received and we are diligently working to resolve the issue.

Although you may email us if there is a problem- i.e records have been requested but has not been received @ [email protected]

BUT please be aware that For your privacy and protection, we cannot transmit medical records by email because email is **not a HIPAA-compliant method** for sending protected health information.

The easiest and fastest way to obtain your records is through your new primary care provider. Simply sign a Medical Records Release Authorization at your new provider's office, and they will fax the request to us. We will securely transfer your medical records electronically in accordance with all applicable laws and regulations.
Printed copies of medical records are also available upon request.
**Record Fees:**
• $1.00 per page for the first 25 pages
• $0.25 per page for each additional page

PUBLIC NOTICE

HEART 2 HEART FAMILY PRACTICE CLOSING ANNOUNCEMENT

It is with a heavy heart that we announce the closure of Heart 2 Heart Family Practice after proudly serving our community for the past five years. Our final day of patient care will be July 2, 2026.

Although patient appointments will conclude on July 2, our office will remain open through August 1, 2026, to assist patients with medical records requests and other administrative matters.

Office Closure Announcement

Dear Patients,

Please be advised that our office will be closed on Friday, April 24th to allow our staff time to be with their families during a period of bereavement. We appreciate your understanding and support during this time.

A provider will be on call to address any acute or urgent medical needs.

Additionally, effective May 8, 2026, our office will be closed every Friday. We kindly ask for your cooperation and understanding as we implement this schedule change.
