Centerpoint Health Centerpoint Health is a community health center serving Middletown, Franklin, and surrounding Ohio communities.

We provide affordable, high-quality care including primary care, behavioral health, pediatric care, and OB/GYN services. The mission of Centerpoint Health is to engage patients in high quality, accessible, and affordable health care throughout Southwest Ohio. As a Federally Qualified Health Center (FQHC) and Patient-Centered Medical Home (PCMH), we are dedicated to improving health outcomes and the overall quality of life in our community. We offer primary care, integrated behavioral health, pharmacy, and outreach services to underserved populations on a sliding fee scale. This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). Centerpoint is protected by the Federal Tort Claim Act Program (FTCA).
