Mansilla Medical Practice at 360 Browns Hill Court, Suite A, Midlothian, Virginia, will close effective June 1, 2023.



A copy of your medical record(s) can be transferred to your new provider. Complete, sign and return a medical record release form as soon as possible, but no later than May 19, 2023. Be sure to indicate how you would like to obtain the medical record(s) and whether you would prefer them in electronic or paper form. We will need 5 business days to retrieve and prepare the records. There will be a charge for paper copies, (50 cents/page for the first 50 pages, 25 cents/page thereafter). WE DO NOT MAIL OUT MEDICAL RECORDS.

After May 31, 2023, you may obtain a copy of your medical record(s) at Mansilla Medical Practice/MCH Health, ATTN: Medical Records, 5309 Commonwealth Centre Pkwy, Suite 403, Midlothian, Virginia 23114. (P) 804/379-3100, (F) 804/379-3200.
