Heritage Park Nursing Center focuses primarily on post-hospitalization transitional care. Our greatest success is the ability to help patients recover and discharge back to home or to a lower level of care! We employ our own therapists and operate a large, fully contained therapy unit. Girolamo came to Heritage Park Nursing & Rehabilitation in Upland, CA, with a clear goal: to regain his strength, endurance, and independence so he could return home to his family.

Throughout his stay, he remained dedicated to both occupational and physical therapy, bringing positivity and determination to each session.

With consistency, motivation, and support from his care team, Girolamo made meaningful progress and returned home with renewed independence.
