If you are unable to stop by the clinic for your athlete’s sports physical, come see us at the Back to School Bash! Slade Dollarhide will be there providing free sports physicals to help your athlete get ready for the school year.

Nurse Practitioner Slade Dollarhide will be at the Community Back to School Bash providing FREE sports physicals in our Medical Mobile Unit!

Join us Friday, July 31 from 5 to 7 PM on the front lawn of the hospital. Please bring any required sports physical paperwork from your school.

Beginning June 30, 2026, cash pay patients seen at our rural health clinics will have an office visit rate of $65. This includes the office visit only. Any additional services, such as labs, injections, or other ancillary services, will be charged separately.
