HOPE Dementia Support is honored to partner with PeaceHealth Southwest Medical Center through the Community Health Hub to provide specialized support for people living with dementia (PLWD) and their care partners during emergency department visits and hospitalizations.

Our team helps reduce stress, confusion, and isolation by providing compassionate, dementia-informed support throughout the healthcare experience. We can:

Support patients and families during emergency department visits by helping staff understand a person's unique needs, preferences, communication style, and routines.
Advocate for person-centered care that promotes dignity, comfort, and the best possible outcomes for individuals living with dementia.
Provide emotional support and guidance to care partners who may be overwhelmed by a medical crisis or hospitalization.
Offer dementia education and resources to families, helping them better understand changes in cognition, behavior, and care needs.
Assist with care planning and transitions from hospital to home, rehabilitation, assisted living, or memory care settings.
Connect families with community resources including support groups, respite services, caregiver education, and local dementia-related programs.
Promote communication and collaboration between healthcare providers, patients, and care partners to ensure the person's voice and preferences remain central to care decisions.
Help identify safety concerns and unmet needs that may affect recovery, quality of life, and caregiver well-being.

Our goal is to ensure that individuals living with dementia and those who care for them receive compassionate support, practical guidance, and access to resources when they need them most.

Together, HOPE Dementia Support and PeaceHealth Medical Center are working to create a more dementia-friendly healthcare experience that supports both patients and their families from crisis through recovery.

A Dementia Minds group is a peer-led circle of people living with dementia who meet regularly to share experiences, learn together, and create educational materials that help the public better understand life with neurocognitive disorders. The groups are part of the National Council of Dementia Minds, the first U.S. nonprofit founded and governed entirely by people living with dementia.

"Explore the new exhibit, “Moments That Made Us,” commemorating our country's 250th Anniversary. The exhibit reflects not only on the people and events that shaped the nation, but also on the local stories that made those national ideals real in everyday life.
