"Why can't you just do it today?"

Fair question. An evaluation and a procedure are two separate services, billed together using something called modifier 25. Some insurers already deny those claims when both happen the same day, and now Medicare is proposing to cut the visit payment in half.

We follow the rules the payers set, and that’s why we sometimes schedule your procedure on a separate day. We know it costs you time, and we don't love it either, but it lets us keep giving you the full care you came for.

Friday afternoon in the “home office,” six years ago today.

A first grader mid-craft project. A second grader in headphones on a virtual lesson. The best dog supervising from my lap. Those days were so challenging, and little did I know what would lie ahead!

What you can’t see in this photo: the practice I was building in between.

Kaufman Allergy was just an idea then - sketched out between “you’re on mute!” and snack requests, with a whole lot of heart and hustle.

Today, those two are thriving middle schoolers, and that amazing dog is deeply missed.

That idea that grew from such uncertain beginnings is now a team of roughly 20, showing up every day for our patients, offering the highest level of care.

We are now offering multiple new services including in-office nasal endoscopy, full pulmonary function testing, remote monitoring for asthma, skin prick and patch testing for allergy evaluation, immunotherapy, and future offerings of OIT and in-office immunoglobulin replacement therapy are both coming soon.

We set out to be a benchmark for excellence in allergy care — and it is a privilege to watch our outstanding team live that out daily.

To every patient and family who trusted us when we were brand new: deeply grateful doesn’t begin to cover it. For all of our patients who continue their care in our practice, our commitment to you is second to none.💚

Most people take their birthday off. Dr. A spent his right here with us, doing what he does best: caring for our patients with the same warmth, patience, and expertise he brings to every single visit.

From his years as a Navy Flight Surgeon to becoming double board certified in Allergy and Immunology and Pediatrics, Dr. A has built a life around showing up for people: for his family, for his patients, and for all of us. Our patients feel it every time they walk into his exam room, and our team feels it every day we get to work alongside him.

What a transformative meeting! Last week I traveled to CA for the EntreMD Business School Vision Retreat - the place where physicians grow in mindset and develop the most impactful and innovative businesses.

⚡️An intentional audit was done on all of the wins of 2026 - all which were carefully planned at last year’s event, and intentionally executed since October of last year.

💡I have my vision cast on my goals for 2027, with a clear roadmap of how to achieve them - RBI daily, Druna to keep me on track, and leaning into my EBS community.

The doctors of EBS are doing incredible things in every single space. I celebrated my 6th year at a new table, in a new mastermind group, and with new vision boards to emulate. I built my second practice, Corneva Clinic, as a deliberate departure from the insurance-driven model, not an adjustment to it.

Shortly after 9/11, I accepted a commission to become a US Naval officer.

After graduation from medical school, I spent 10 years on active duty as a Navy physician — internship, residency, fellowship training, several years in academic medicine, and a deployment aboard the hospital ship USNS Comfort.

That chapter taught me organizational leadership, teamwork, and refined my ability to serve others in my greatest capacity. Those qualities translated directly to my life after the military — and helped shape the clinician and practice owner I am today.

Some of the most important work happens outside the exam room.🤍

For years, I’ve watched my most medically complex patients build extraordinary teams of specialists around themselves — an immunologist, a rheumatologist, a cardiologist, sometimes a neurologist and a physical therapist too — and still have no one managing their hormones through perimenopause and menopause. Not because anyone failed them. Because every visit was already full.

I know this patient well. She’s often done more research on her own condition than the physicians she’s seen. She’s living with immune dysregulation, immunodeficiency, or autoimmunity — sometimes more than one at once — and she’s learned to advocate for herself because no one else has connected the whole picture. She’s not falling through the cracks between two doctors who aren’t talking. She’s falling through the cracks in a medical system that is completely fragmented.

The hormonal piece keeps getting pushed to next time, though next time never comes. I built something to close that gap.
