What patients always ask: what helps me heal faster? The honest breakdown:

Real: ice/cold compresses first 48h, head elevation at 45° the first week, hydration, and protein (your body rebuilds tissue from it).

Modest: arnica and bromelain — evidence is mixed, probably help bruising a little, won't hurt.

Bro-science: lymphatic massage too early (disrupts healing), mega-dose vitamins (you excrete the excess), "healing accelerator" supplement stacks (save your money).

The biggest levers are the boring ones.

Neck and chin liposuction can sharpen a jawline — in the right patient. Usually younger, good skin elasticity, isolated fullness under the chin.

Sold to a patient over 45–50 whose skin has lost elasticity, it backfires. Remove the fat, the skin doesn't bounce back, and now there's loose hanging skin with nothing filling it out. Skinnier, not younger.

Past your 40s, a defined jawline usually needs deep tissue repositioned and skin redraped — a lift, not lipo alone.

As we age, the distance between the nose and the top of the lip lengthens, and the upper lip rolls inward. Patients add filler — and for a while it helps.

But filler doesn't shorten that distance. Past a point, volume on a long upper lip just looks puffy and projects forward (the "duck" look). The problem is length, not volume.

A lip lift removes a small strip of skin under the nose and physically shortens it — permanent, 45 minutes, in-office. If lip filler's stopped looking right, the answer might be a lift.

The dramatic part is surgery. But the month before quietly determines a lot of your result. What I tell every patient:

1. Medical skin care (retinoid, vitamin C, sunscreen) at least 4 weeks out — heals better, scars less.
2. Stop blood thinners your physician approves stopping (aspirin, ibuprofen, fish oil, vitamin E). Always clear with your prescribing doctor.
3. No alcohol for a week before — it worsens swelling and bruising.
4. Dial in sleep and blood pressure — surgical bleeding tracks with BP.

None of it costs much. All of it shows up at 6 months.

Where the marketing oversells: "it reaches deep enough to lift like a facelift." It doesn't. These needles safely reach the dermis and superficial fat — not the deep structural layer that holds your face up.

Excellent skin-quality tool. Not a substitute for repositioning deep tissue.

Buccal fat is the rounded pad in your lower cheek. TikTok made removing it trendy. Here's what nobody tells you:

Your face loses fat as you age — everywhere. The buccal fat that looks "full" at 25 is what keeps you from looking gaunt at 45. Remove it young and you accelerate your own aging. Reversing it means fat grafting back exactly what was taken out.

I turn down most buccal fat consults. Genuine candidates are rare. For everyone chasing a trend, it's a one-way door.

Ultherapy, Sofwave, HIFU, RF — real technologies that modestly tighten skin by triggering collagen. For early, mild laxity in your 40s, they can buy you a few years.

But "non-surgical facelift" is false advertising. None of these reposition deep tissue or remove skin — which is what a facelift does. Sell that label to a 60-year-old with real jowling and they spend $5–8K to see almost nothing.

Energy devices are skin-quality tools. Don't buy them as a substitute for surgery you actually need.

Thread lifts are marketed as a "lunchtime facelift." For the right patient — late 30s to early 40s, mild laxity, good skin — they're a reasonable subtle tweak.

For everyone with real jowling or neck laxity, they pull on tissue too heavy to hold. Result lasts about 6 months, and the barbs can leave dimpling I have to work around later.

If you've been offered threads for significant sagging, get a second opinion before you spend.
