I’ve given daily Tadalafil to a number of patients over the last few years and in my very limited and anecdotal experience, it has proved helpful. Would love to know if you have any experience or any opinions on the subject?

Not trying to inflame anybody. I understand the rationale behind NP thyroid and if you want to dabble in it, that’s fine. But be careful about causing iatrogenic thyrotoxicosis. It’s real we are seeing it and it can get you in trouble. i’m also a big believer in testosterone, but it is misused and abused very frequently and you don’t want to fall into that trap.

for the purposes of the paper, there was some differentiation between premature ovarian insufficiency (under age 40) and early menopause(under 45) . But for the purposes of thinking of this clinically, to my mind what really matters is that women who are young whwn their menopause journey begins need to be far more aggressively treated and monitored

There needs to be a plan for monitoring your bones and your cardiovascular and sexual well-being. This is not one of these situations where you just set it, and forget it with MHT.

I would be aware of these risks and think about this as you plan your care with your medical team. This includes primary care.

Janyce had ovarian failure at age 43 post hyst. She is petite with a small frame and a family history of osteoporosis. Believe me, this hits home for me. And if it hits home for you, find somebody who will help you navigate this.

I get it. Somebody takes a pellet course and thinks they are an expert. Or somebody does not want to invest in all the tools required to be great at their craft. It’s hard; it’s expensive; it’s a commitment.

But if you want to be great at anything, you need to invest in your training and grow in your skill set.

This is true in music and this is true in menopausal Care.

if you don’t know why one paradigm might be better in certain situations than others; if you only offer simplistic protocol based approaches and apply them to every single patient. Then you only have one guitar. And you really don’t have a place in the band as far as I am concerned.

There’s a lot of shady stuff out there. Everyone should admit this. I have no problem calling out physicians who are abdicating their role in helping to provide care for women who need help with hormonal issues; I have no problem calling out clinics that have unscrupulous medical or business practices

Steer clear of the doctors who won’t learn hormone therapy and steer clear of the people who take a weekend course in hormones, and now are doing unscrupulous things
