Community HealthCorps is the largest health focused national AmeriCorps program. Our program promotes healthcare outreach, education, and support to individuals of the Heart of Texas Community Health Center (dba Family Health Center) and local community through various clinical programs, health seminars, and community sponsored events. Service Descriptions include but are not limited to: Health Li

teracy, Diabetes Self-management, Nutrition Counseling, Medicaid Part D and insurance assistance and screenings, dental promotions, women's health outreach, and other patient advocacy activities. Members also participate in providing community awareness events, presentations, health fairs and seminars upon request from local agencies. Members conduct these services throughout the Family Health Center's 12 sites and various collaborating agencies such as CareNet Pregnancy Center, Baylor Campus Kitchen's and the Advocacy Center for Crime Victims and Children.
