Last week over 80 elementary school children took the time to make Valentine’s cards and gifts for our residents. No connection between the after school program and La Casa Via Transitional Care Center. Just an act of random kindness. Thank you Natalie with Murray Elementary School in Dublin for facilitating the ultimate message of love— giving of the heart. May we carry this through every day of the year. ❤️

One of our lovely 90 year old ladies needed a blood transfusion. Scheduling this medical procedure at John Muir’s Infusion Center is the preference when circumstances permit. No family in the area so accompanied her this early Saturday morning to get her settled in. Far better option than sending her through the Emergency Department. Always providing the highest quality of care and support for our patients at La Casa Via Transitional Care Center. ❤️

One of our youngest success stories— orthopedic patient in his 30’s afflicted with bilateral hip osteoarthritis with ankylosing spondylitis. Eleven month skilled nursing and rehabilitation stay after two hip replacement surgeries by Dr. John Kronick. Two months of skilled physical and occupational therapy x5/week after each hip replacement surgery (left hip July 2023, right hip December 2024). Transitioned to our RNA program with twenty minute sessions x3/week and bonus gym time per patient request. Goal of ambulating without any assistive device achieved! Admission date: July 13, 2023. Discharge date: June 21, 2024.
