For a deep, fixed nasolabial/laugh line, subcision followed by a biostimulator or filler can be a logical combination because they address different components.

A practical sequence is: release → assess → rebuild. Subcision releases fibrotic/tethering attachments beneath the fold. After release, reassess how much of the line remains. Then choose the injectable based on what is missing: HA filler when you want immediate, precise structural correction; CaHA (Radiesse) when you want some immediate support plus collagen stimulation; or PLLA (Sculptra) when the goal is gradual, more diffuse collagen remodeling rather than directly filling the crease.

One important distinction: a true nasolabial fold often isn’t simply a tethered scar. It can be driven by midface volume loss, maxillary/pyriform deficiency, and descent of the cheek, so aggressive subcision directly under every laugh line isn’t necessarily appropriate. In those cases, restoring the pyriform/midface support first may soften the fold without putting much product directly into it.
