We follow the original CDC guidelines to keep our patients as safe as possible. Please be mindful of who you are spending time with if you have an infant who cannot receive the MMR vaccine yet. If you are going to be traveling with an infant, please let us know and we can discus if early vaccination would be an option.

JCPHS, along with the NYS Department of Health, would like to inform our community that there is a recent increase in measles cases in rural areas across the state.
⏺ There are no confirmed measles cases in Jefferson County.
⏺ Cases elsewhere in the state have been linked to both travel outside of NYS and local community spread.

The MMR vaccine is the best protection against measles. If you or your child are not up to date, now is the time to contact your healthcare provider.

RN Case Manager, Home Care
Join our interdisciplinary team providing and coordinating end-of-life care, symptom management, and support for patients in their homes and nursing home settings. This full-time position requires a valid New York State RN license and the ability to travel throughout Jefferson County as needed.

LPNs, Hospice Residence
Full-Time & Part-Time Evenings
Full-Time & Part-Time Nights

Our Residence LPNs work alongside a uniquely talented interdisciplinary team focused on symptom management, comfort, and end-of-life care. New graduates with a valid New York State LPN license are welcome to apply.
