Our friends at Randolph EMS shared this. This is such a great idea to have already prepared in case of an emergency. You should print and complete this for each person in your household. Keep it somewhere that is easy to find and be sure the other members of your household know where to find it as well. It would also be a good idea to keep a copy of your insurance card with it! Thank you Randolph EMS for sharing this with our community!
🚑 BE PREPARED BEFORE AN EMERGENCY HAPPENS 🚑
During a medical emergency, every second matters. Sometimes a patient may be unable to answer questions, family members may not be present, or the stress of the situation can make it difficult to remember important medical information.
That’s why Randolph Emergency Medical Services encourages every household to complete an Emergency Medical Information Sheet for each person living in the home.
This sheet gives EMS quick access to important information such as your medical history, current medications and dosages, allergies and reactions, medical implants or devices, previous surgeries/procedures, emergency contacts, baseline health status, and advance directives.
📍 Once completed, keep it somewhere EMS can easily find it. A refrigerator, clearly marked folder near the front door, or another consistent and visible location can work well. Let everyone in your household know where the information is kept.
Remember to review and update the form anytime medications, medical conditions, allergies, or emergency contacts change.
Having accurate information available can help emergency responders better understand your medical history when you may not be able to tell us yourself.
