✏️Floxed* and trying to remember every physical symptom later is a losing game....
We co-created a free, ultra detailed, multi page symptom tracking form you can download and start using today, which includes issues reported from the floxed community we have received, viewed on the support groups over the last 11 years, as well as our own personal experiences. You never know when this might come in handy.
This form is designed to help as a:
✅ Resource reminder of your top complaints - visit prep
✅ Track your symptoms over time (important)
✅ Identify possible triggers
✅ Use body diagrams to map affected areas
Due to the complexity of FQAD** (fluoroquinolone associated disability) hitting many systems within the body, new symptoms can possibly keep appearing daily, weekly or monthly, especially in the first year or so - most of which appears to the the acute stage. In addition, many report new symptoms appearing after this time period. This form is a good way to stay organized - for your own records, and could be useful for when it comes to approaching your doctor for the ICD-10-CM medical code for 'fluoroquinolone adverse effects' on your next visit (medical code info viewable on the link just below).
