In the early 1990s, smoothing a patient's forehead required one of the most violent trade-offs in aesthetic surgery. To lift two eyebrows, surgeons had to make a massive ear-to-ear cut across the top of the skull, peel back the scalp, and strip away skin.
The procedure worked, but the clinical cost was immense: permanent scalp numbness, hair loss along the scar line, and a distorted hairline. Surgical dogma insisted that real structural elevation was impossible without cutting open the entire head.
Dr. Luis Vasconez and Dr. Grady Core at the University of Alabama at Birmingham refused to accept that premise.
Building on UAB’s legacy of endoscopic innovation, they asked a fundamental question. Why strip the entire scalp when you can achieve the exact same anatomical release through keyholes?
Between 1992 and 1994, Vasconez and Core shattered the status quo. They replaced plastic surgery’s longest incision with five sub-centimeter portals hidden behind the hairline.
Guided by an endoscope beneath the scalp, they released the arcus marginalis and treated the underlying muscles under direct visualization. The ear-to-ear scar vanished. Scalp sensation was preserved. Recovery times dropped overnight.
Vasconez used UAB as an international incubator, mentoring future leaders like Dr. Renato Saltz, Dr. Nazim Çerkeş, and Dr. Jorge de la Torre to carry endoscopic facial techniques across continents.
Yet the true soul of his career extended far beyond technical milestones. For over thirty years, Vasconez returned to Ecuador, performing more than seventeen hundred free surgeries for underserved children. He proved that while technical innovation redefines a field, human care defines a legacy.
