56-year-old with a previous L2-L5 fusion who came back with progressive deformity, worsening back pain, radiculopathy, and a T9-10 disc herniation on top of it.

We took him from T9 to the pelvis — three-rod construct, iliac fixations, osteotomies — and corrected both his coronal and sagittal alignment. PI-LL mismatch down to 3 degrees.

He went from walking hunched forward to standing tall. He’s doing great.
