When the hip is unstable, the bone pays the price.

Why bend the needle?

The bevel on a needle tip is not just for piercing. That angled edge creates a directional bias, so the needle naturally curves toward the opposite side of the bevel as it advances. Putting a slight bend in the tip amplifies that effect and gives me the ability to steer around structures instead of driving straight through them.

Two antiseptics, two completely different techniques, and there is a reason for each.

Iodine works by sitting on the surface and releasing free iodine over time, so it goes on in concentric circles moving outward. That direction matters because every pass travels away from the cleanest point, so you never drag bacteria back toward the site you are about to puncture.

Chlorhexidine works differently. It binds to the skin and keeps killing bacteria for hours after application, but it has to reach the deeper layers where organisms actually live. That takes friction, so it goes on with a back and forth scrub rather than a gentle circle.

The syringe is a pressure instrument, not just a container.

How fast you pull back the plunger changes the physics inside the barrel. Slow, controlled tension creates minimal negative pressure and yields clean venous blood. Rapid aspiration generates a much stronger vacuum that pulls cellular material along with it, which is exactly why aggressive suction belongs in a marrow draw and not a superficial one.

Here’s how we take the edge off.

Right before the sedation shot, my nurse sprays the shoulder with a cold numbing agent. It distracts the nerves and dulls the surface so the injection feels like almost nothing. Little things, big comfort.

Feel is everything.

As I pass the needle through the periosteum and meet bone, tapping the bone marrow trocar tells me everything. That change in resistance confirms I am purchased in and exactly where I need to be before I draw marrow.
