✨ Why A Neck Lift May Not Be Enough ✨

Many patients, women and men, come to me certain that their neck is the problem.

They see a wattle and loose skin under the chin, double chin and fullness in the neck, or a jawline that no longer looks defined. Because that is where the change is most visible, an isolated neck lift feels like the obvious solution.

But I do not plan surgery around the area a patient points to.

I plan it around where the aging actually begins.

🧠 The face and neck often age together. As the cheeks and lower face descend, it creates jowling, and adds heaviness to the jawline and neck. The neck may be where the problem becomes most noticeable, even when the aging actually started above.

This is why tightening the neck or a liposuction alone is not enough, especially when there is enough laxity and loose skin. The tissues that have descended have to be lifted and repositioned. The excess skin needs to go somewhere.

Two patients can dislike the same part of their neck and still need completely different surgical plans.

My role is to educate and not simply to perform the procedure a patient requests. It is to determine whether that procedure addresses the source of where the aging or change is coming from.

Most patients see the consultation, the day of surgery, and the recovery.

What they do not see is how much of the operation has already begun before I ever enter the operating room.

Before every surgery, I review the patient in front of me, not simply the name of the procedure we are performing.

I study her anatomy, photographs, existing asymmetries, prior treatments, and the procedures we are planning together.

That review shapes the surgical plan.

And the plan shapes the room.

The instruments, equipment, positioning, and sequence of the operation should reflect the exact surgery we are preparing to perform.

My team also needs to understand that plan clearly.

We know our roles.

We communicate consistently.

We anticipate what may be needed next.

That preparation reduces avoidable distractions once surgery begins and allows us to remain focused on the patient in front of us.

A calm operating room is rarely accidental.

It is usually the result of decisions and communication that happened long before the patient arrived.

Many of these details are invisible to the patient, but they support the safety, comfort, and attention surrounding her care.

Preparation creates a more deliberate surgical environment where the surrounding details support the work being performed.

Preparation is not separate from the surgery.

It is part of the surgery.

✨ My Endoscopic Facelifts Are Still Deep Plane ✨

One of the biggest misconceptions I hear about an Endoscopic Facelift is that smaller hidden incisions must mean a smaller or weaker lift. In my approach, that is not how this procedure works.

The incision is what patients notice first, but it is not what determines the strength of an endoscopic deep plane facelift. What matters is what those smaller incisions still allow me to reach underneath the surface.

In an endoscopic deep plane facelift, I am still working in the deeper layers. The lift is not coming from simply pulling skin from the outside. The fallen cheek fat pads and jowl fat pads are lifted directly, because those are the tissues that create heaviness around the mouth, cheeks, and jawline when they descend.

Those tissues are then secured to a stable structure, which in my approach is the deep temporalis fascia. That is why the strength of the lift is not coming from the incision being smaller. It is coming from directly lifting the fallen tissue and fixing it to something stable.

This is also why smaller incisions do not automatically mean easier downtime. Recovery can be similar to an open deep plane facelift, and some patients can even have more swelling through the center of the face because that is where the deeper fat pads are being directly lifted and secured.

So yes, hidden incisions are part of the benefit of my endoscopic deep plane facelift, but it’s not the whole story.

✨ Why Jowls Aren’t Just Loose Skin ✨

Here’s the part most people get wrong about jowls.
They’re not caused by extra skin. They’re caused by displaced support.

Jowls form when the deeper structures of the face lose strength over time. The fat pads that once sat higher in the face begin to descend, settling along the jawline and creating heaviness in the lower face. This happens because of loss of collagen and structural support, and the fat pads descending. This is why skin tightening treatments or laser resurfacing alone can’t lift the jowls. Those treatments work more surface-level. Jowls are a structural problem, not a skin-quality problem.

🧠 To truly address jowls, the support underneath has to be lifted. In a deep plane facelift, I work below the SMAS layer to release and reposition the ligaments that hold the lower face. This allows the fat pads to be repositioned to where they naturally belong, above the jawline. The result is a definition without a tight or artificial look.

Understanding why something is happening is key and guides the right solution.

✨ He Lost 80 Pounds - Why A Neck Lift Is Not Enough ✨

This patient of mine had a deep plane facelift and neck lift in his mid 40s. He had a neck wattle he wanted to address. He always noticed that a double chin and neck wattle runs in his family. As his weight came off, the excess loose skin became even more prominent and his face sagged even more. He regularly found himself resting his chin on his knuckles or buttoning his shirts higher to cover the area.

This was not simply a matter of removing skin with an isolated neck lift. The deeper tissues have descended from the upper face - the cheeks down to the jawline, and neck, so these areas needed to be treated as one connected facial unit. If you are choosing a Facelift based on the name alone, you may be missing the part that matters most.

A lot of patients come into consultation confused by all the terminology: mini facelift, SMAS facelift, deep plane facelift, endoscopic facelift, and branded facelift names. That confusion makes sense, because not every surgeon uses the same language, and sometimes the name of the procedure sounds more clear than what is actually being done underneath the skin.

That is why I tell patients not to stop at the name. The more important question is: what layer is being lifted, where is the tension being placed, and are the tissues causing the sagging actually being addressed?

If a lift relies mostly on the skin, too much tension can end up showing at the surface. If a lift goes deeper but does not fully release the structures holding the fallen tissue down, the correction may still be limited. A deep plane facelift is different because it works underneath the SMAS layer and releases the deeper attachments, so the fallen cheek and jowl tissues can be lifted more directly.

This is why facelift names can be misleading. The name does not always tell you where the real work is happening. The question needs to go beyond “Do you do this named lift?” The better question is whether the layer causing the heaviness and the root cause of sagging are actually being addressed and lifted in a way that will look natural.

Different surgeons may use different words, but what matters most is understanding what is being lifted, how it is done and whether that plan matches your anatomy, goals and your aesthetic vision of what a natural outcome looks like.

🧠 A facelift can restore support and address the deeper structures in the face and neck, but it does not replace volume loss or improve skin texture by itself. When hollowing, tired appearance, aging around the mouth, and skin quality are also part of the concern, a more complete plan may include fat transfer, lip lift, and CO2 laser resurfacing. At 9 months, swelling has softened significantly, while subtle refinement can still continue toward the one-year mark.

A lot of people blame dark circles on sleep, allergies, or a skin issue, but the lower eyelid anatomy may be the real reason.

When the area under the eyes looks darker, the instinct is to treat the skin itself with more sleep, eye creams, allergy treatments, or concealer. But for many patients, the darkness is not always true pigment. Sometimes, it is shadowing.

That shadow can happen when there is hollowing, volume loss, thin skin, visible vessels, or lower eyelid fat pads. When the lower eyelid and cheek junction loses support, light can hit the area unevenly. The eye bag may catch light, while the hollow underneath creates a shadow that makes the under eye look darker.

That is why the solution depends on the cause.

If the issue is hollowing, fat grafting may help restore support and soften the shadow. If lower eyelid bags are contributing to the contour, lower eyelid surgery may be needed to reposition the fat and smooth the transition over the rim of the bone. Sometimes both are needed.

This does not mean allergies, pigmentation, or skin quality can never play a role. But when dark circles do not improve with the usual fixes, the structure underneath has to be evaluated.

The mistake is treating every dark circle like a surface problem.

For many patients, the more important issue is the contour between the lower eyelid and the cheek. When that contour is smoother, the shadowing can improve and the under eye can look more rested and rejuvenated.

This Fourth of July weekend was exactly what I needed - a chance to slow down, recharge, and spend quality time with some of the people I care about most. ❤️

As much as I love what I do, I’ve learned that showing up as my best starts with taking care of myself, too. Rest isn’t stepping away from your purpose. It’s part of it. Taking time to laugh with friends, enjoy the sunshine, and simply be present fills my cup in ways that make me better.

I also picked up golf again and finally committed to taking lessons. ⛳️ If you know me, you know I’m a little stubborn - in the best way. I don’t like giving up on a skill just because it’s difficult. I genuinely enjoy the process of learning, just doing it, and getting better.

That mindset is something I carry especially with my work as a facial plastic surgeon. Mastery doesn’t happen overnight. It comes from curiosity, humility, repetition, and the willingness to keep learning, no matter how much experience you have. Every technique can be refined. Every detail can be improved. Every patient deserves that level of commitment.

This weekend reminded me that growth is also about creating space to recharge, surrounding yourself with people who matter, and staying open to learning something new...even if it means hitting a lot of less-than-perfect golf shots along the way!

✨ Endoscopic Deep Plane Facelift, The Part Patients Miss ✨

Most patients think the best facelift is the one with the smallest incision. That is not actually the part that matters most.

What patients often miss is that the best approach has to match what is being addressed. An endoscopic deep plane facelift can be a beautiful option because the incisions can stay hidden in the hairline and behind the ear, but that does not automatically make it the right choice for every face.

The real question is what happens after the deeper tissues are lifted. If there is too much loose skin left over, that skin still has to go somewhere. And if it cannot re-drape smoothly, it can bunch in front of the ear and make the result look more operated instead of more natural.

That is when an open deep plane approach may actually be the better choice. Not because it is more aggressive, but because it allows the extra skin to be removed and the incisions to be placed in natural areas around the ear and hairline where they can still heal well.

The goal is choosing the approach that gives your face the best and most natural result.
