The Plan has programs for individuals needing a standard Medicare program, special programs for individuals with both Medicare and Medi-Cal (Dual Special Needs Plan or D-SNP) and specialized Chronic Care programs for individuals diagnosed with any of the following chronic conditions: Diabetes, Congestive Heart Failure, Dementia, Major Depression, Schizophrenia, Schizoaffective Disorder, Bipolar Disorder, and Paranoid Disorder. The plan is licensed to service members who live in Los Angeles, Orange, Riverside, San Bernardino, Kern, and Kings Counties. Brand New Day has developed a number of programs to support our Members, their family and caregivers, and the member’s Family doctor and specialists.
