Is a BMI cutoff and a questionnaire enough for medical weight loss care?

Direct to consumer weight loss programs are everywhere right now, and there are real reasons why: many insurance plans still do not cover GLP-1 medications for weight loss, compounded versions filled the gap during the shortage, and brand name drugmakers now sell FDA approved versions directly to patients at self-pay prices to stay competitive.

But convenience and comprehensiveness are not the same thing.

BMI is a screening tool. It cannot distinguish body fat from muscle, and it says nothing about insulin resistance, thyroid function, sleep apnea, hormonal changes, medications or the other factors that influence weight and metabolism.

And with the FDA declaring the shortages resolved, large-scale 503B compounding of copies has ended. Patient-specific 503A compounding continues, but those products are not FDA approved and quality oversight varies.

A GLP-1 can be a powerful tool. It should not automatically become the entire plan.

Most people diagnosed with colon cancer had no symptoms beforehand.

That is why screening matters.

March is Colon Cancer Awareness Month, an important reminder that colorectal cancer is one of the most preventable cancers when detected early.

Unfortunately, many people delay screening because they feel healthy.

The reality is that early colon cancer often develops silently.

When should you get screened?
Current guidelines recommend colon cancer screening starting at age 45 for adults at average risk.

You may need earlier screening if you have:
• A family history of colon cancer or polyps
• Inflammatory bowel disease such as Crohn’s disease or ulcerative colitis
• Certain genetic conditions

What screening options are available?

There are several effective ways to screen:
✔ Colonoscopy (gold standard, typically every 10 years)
✔ FIT stool test performed annually at home
✔ Stool DNA testing every 3 years
✔ CT colonography every 5 years

These screenings can detect precancerous polyps and prevent cancer before it develops.

A simple reminder Preventive care is one of the most powerful tools in medicine.

If you are 45 or older and have not been screened, this month is a great time to start the conversation with your physician.
