Thanks for coming in Sofia! Sofia is actually our neighbor and is a fellow business owner as well. She owns a meal prep service and makes different meals each week that are fantastic. The meals are labeled with calories and macros and make life so much easier if you’re busy. I use the service myself and it’s the only meal prep company I’ve ever used where the meals are actually delicious 😂

Now, a quick insight about this visit. While trying to manipulate Sofias neck, I could feel she was guarding a bit during the thrust of the movement. I don’t thrust very hard and if the manipulation doesn’t work once and it isn’t there, I don’t force it. If you’ve done enough of them, you’ll know when to stop trying if the first attempt doesn’t feel right. That said, we opted for soft tissue therapy because it practically does the same thing, it just takes a bit longer. There’s more than one way to provide a therapeutic effect 🤝

Here’s a session with my patient seeking treatment for his painful shoulder due to adhesive capsulitis and RCRSP.

Some takeaways from the video:
1. My “flow” for care is dependent on the flow of the conversation and the position of the patient. When the patient was on his back, we did all the things we could do when he was in that position. When it comes to the order of MT and exercise, which comes before or after is probably inconsequential so my treatment blends both together.
2. Throughout treatment I’m always asking questions. For example, the patient told me pressing movements were fine, but I wanted to see for myself so I checked them. This brought up a question the patient had about pressing being “safe”. I assured him that pressing was safe so long as he doesn’t have any long lasting debilitating pain afterwards and reassured him so that he can stay active.
3. Your rehab does not need to be long or complicated. It just has to work. I’ve treated Greg for the same issue before, so we have some idea of what movements give him relief and more ROM. In the video he goes on to say he did some movements in the HEP he was given and that they helped him retain some ROM and strength, but later on (not on video) he admits he probably hasn’t been doing them as much as he should be because he is often busy and forgets.

4. Yes, I said “loosen up” the tissue. Greg knows through previous discussions that I am not loosening his muscles physically by rolling them out like pizza dough so don’t call the nocebo police it’ll all be okay.

Hi friends, here’s another 30 min session with a client distilled down into 1.5 minutes.

This gentleman presented with cervicogenic vertigo and concurrent neck pain with headaches. At the beginning of the video we spoke for about 10 minutes discussing the alleviating and aggravating factors of his condition. The patient states that opening and closing his jaw, contralateral rotation of the head/neck and cervical extension all make his pain worse while a TENS unit temporarily makes his pain better. Upon further questioning the patient states he only feels vertigo at night before bed and it only lasts a few minutes at a time. The patient then went on to reveal that exercise also makes his pain and his vertigo better and stated “if I do some neck exercises for about 30 minutes, it typically goes away completely, but not this time”. The patient also reveals that cervical manipulations have helped to completely eradicate his vertigo when he is in the midst of an episode and that he has had a physical therapist perform the Eply maneuver many times to no avail. The patient has only ever taken Tylenol for his headaches, neck pain and vertigo and states it does not work. This gentleman has no other comorbidities such as diabetes, high blood pressure, etc. He has also never seen an ENT or Neuro for his condition and so a referral was also made after this visit for a follow up with a medical physician.

In this session we do some SMT, some deep tissue work and some cervical rehab. Not shown here due to the camera angle (my bad) are the resisted cervical rotations with an exercise band. Hope you enjoy
