The deciding factor isn't the size of the original cavity, but how much healthy structure remains after cleaning it out. If the tooth still has enough healthy structure to handle chewing, a filling works well. If what's left is too thin or weak, or the tooth still has years to go before falling out naturally, a crown protects it better.
That's why two kids with cavities can get two different treatments each tooth is judged on its own.
