📚 Back-to-School Reminder ✏️

As the new school year approaches, we know many families will need school medication forms and Seizure Action Plans completed before classes begin.

To help ensure everything is ready on time, please remember that our office requires 48–72 business hours to complete these forms. If your child will need school paperwork, please call our office or have your child’s school fax the forms to us as soon as possible. Our fax number is 316-686-9797.

Our team will be working through these requests all next week. Please also note that one of our providers will be out of the office August 10–12, which may result in longer turnaround times. To help avoid delays, we encourage you to submit any forms before that time whenever possible.
