What does the research actually say about full-fat vs. low-fat dairy?
I went looking for the answer back in 2016, for my dietetic internship. I read every study I could find from 2009 to 2016, built a poster out of it, and landed on a conclusion that felt a little bold at the time: there was no evidence that choosing low-fat over full-fat dairy was better for your health.
That poster (slide 3) went to the Massachusetts Dietetic Association meeting. I was one of a small number of dietetic interns in the state chosen to present research that year, which is why I have hung onto a very large, very taped-together sheet of paper for a decade.
Ten years later, the conclusion has held up.
The PURE study (The Lancet, 2018) followed 136,384 adults in 21 countries for about nine years. People who ate more dairy had fewer deaths and fewer serious heart problems, and that stayed true even among the people eating only full-fat dairy.
Because we are all bad at remembering what we ate, other researchers skipped food diaries entirely. They tested the blood of 63,682 people across 16 studies and measured dairy fat directly. The people with the most dairy fat in their blood had roughly 20 to 35% lower risk of type 2 diabetes, and the link was even stronger in women.
The honest takeaway is less exciting than "full-fat wins." Milk, yogurt, and cheese come out either neutral or slightly good for your heart, with or without the fat. Fat was never the problem.
So why do I still point most women, especially in the childbearing years, toward full-fat?
Because the fat is how the nutrients get in. Vitamins A, D, E, and K2 need fat to be absorbed, so when you take out the cream, you take out their ride. Because fat slows digestion, which steadies blood sugar and keeps you full longer. And because in the Nurses' Health Study II, women who ate at least one serving of full-fat dairy a day had a 27% lower risk of trouble ovulating, one of the most common causes of infertility, while women eating two or more servings of low-fat dairy a day had close to twice the risk. That kind of study shows a pattern rather than proof, but it is worth knowing if you have spent years being told to skim everything.
NOW, ABOUT QUALITY.
This is the part I get asked about most, and it is where I tell clients to look for grass-fed A2 dairy when they can.
Most milk in this country comes from cows kept indoors and fed a mixed ration of grain and stored feed. Cows on pasture eat living plants, and researchers can measure the difference in the milk. Pasture-fed milk consistently carries more omega-3 fats and more CLA, less of the saturated fat that raises cholesterol, and more vitamin A, vitamin E, and beta-carotene. It also carries more of the plant compounds that come from grazing a variety of forage instead of one uniform ration. One 2026 analysis put organic grass-fed milk at about 88 mg of omega-3 per cup, compared to 25 mg in regular milk.
You can actually see this. The yellow in grass-fed butter is beta-carotene from fresh grass. The difference is biggest in spring and early summer and smallest in winter, when even pastured herds are eating stored hay.
A2 is a different question, and I want to be straight with you about it. Most cows make two forms of a milk protein called beta casein, A1 and A2. Breeds like Jersey and Guernsey make mostly A2. When A1 is digested it releases a compound called BCM-7 that appears to slow the gut and irritate digestion in some people. Every systematic review agrees A2 milk tends to be gentler on the stomach. Those same reviews also agree there is no solid evidence that A1 milk causes disease, and that a good share of the friendliest studies were funded by the company that sells A2 milk. So if regular dairy bothers you, A2 is genuinely worth trying. I just would not buy it expecting it to do more than that.
And if none of this is in the budget, buy the whole milk yogurt that is on sale. The difference between grass-fed and regular is real. The difference between A2 and conventional is real if your stomach is sensitive. But the difference between full-fat dairy and no dairy at all is bigger than either one. Grass-fed A2 is the upgrade. It was never the price of entry.
One last thing. In January 2026, the new Dietary Guidelines for Americans (covering 2025 to 2030) listed whole milk and full-fat dairy as options. That is a real shift from what I was taught. The cap on saturated fat, 10% of daily calories, stayed the same, and dietitians are still working out how those two pieces fit together.
