Valid Registered Nurse's license from the Commonwealth of Massachusetts,

Valid CPR certificate,

Essential Functions:

Conducts home visits as needed including clinical assessments, initial assessment and re-assessments. Develops Care Plans that are appropriate to meet the member's needs
Complete and prepare all necessary assessments, data, forms and care plans as required
Keeps updated progress notes in accordance with EOEA documentation standards.
Works closely with care managers, consumers, and caregivers to coordinate services.

Join us on January 15, 2023, as Tết in Boston celebrates the lunar new year with live performances and activities such as the lion dance, martial arts, music, a fashion show, fun games, raffles, and more throughout the Day Festival!
