🎱 Men's Club at Its Best! 😊

Our Men's Club at Woodbury enjoyed a great time together with a few friendly games of pool. There was plenty of laughter, conversation, and just the right amount of competition as everyone showed off their skills.

It's always wonderful to see residents coming together to enjoy each other's company and make lasting memories. 💙

We are proud to announce that Atlas Post Acute at Woodbury Country Club has earned the American Heart Association Skilled Nursing Facility Heart Failure Certification. ❤️

This certification recognizes our commitment to providing evidence-based care for residents living with heart failure.

It evaluates skilled nursing facilities against rigorous, science-based standards for program management, patient and caregiver education, care coordination, clinical management, and continuous quality improvement.

Nearly 1 in 4 heart failure patients are readmitted within 30 days of discharge, and approximately half are readmitted within six months.

About on quarter of these readmissions may be preventable.

This certification supports our ongoing efforts to help reduce preventable hospital readmissions while enhancing the quality of care our residents receive.

We remain committed to delivering exceptional, resident-centered care and continually advancing the quality of services we provide.
