Why Don’t Pediatricians Routinely Screen for Retained Primitive Reflexes Later in Childhood?

During a recent Lunch & Learn with a pediatric medical group, I had the opportunity to ask an important question:

“Why aren’t primitive reflexes routinely screened or tested beyond early developmental milestones, especially when some children continue to struggle with challenges that may be associated with retained reflex patterns?”

The response was very insightful. The physicians shared that, during their medical training, primitive reflexes were primarily taught as developmental milestones—something to monitor during specific age ranges in infancy and early childhood. If reflexes appeared integrated within those expected developmental windows, ongoing screening was not typically emphasized.

However, in clinical practice, many children, teens, and even adults continue to experience challenges that may be associated with retained primitive reflex patterns, including difficulties with:

🔹 Balance and coordination
🔹 Fine motor skills and handwriting
🔹 Attention and focus
🔹 Sensory processing differences
🔹 Emotional regulation
🔹 Postural control and fatigue
🔹 Learning challenges

Primitive reflexes are automatic movement patterns that support early survival and development. As the nervous system matures, these reflexes typically become integrated, allowing more advanced motor, sensory, and cognitive skills to develop.

When integration does not occur as expected, some individuals may continue to rely on these early movement patterns, which can impact how efficiently the brain and body communicates.

This conversation highlights an important opportunity for collaboration between medical providers, occupational therapists, educators, and families: continuing to look beyond developmental milestones and asking, “What is happening with this child’s nervous system today?”

At BalanSense-Therapy, LLC, we service children and adults. We look at the whole nervous system—supporting regulation first, and then sensory processing, motor development, and connections between the brain and body become easier to integrate helping individuals reach their full potential.

✨ Sometimes the missing piece is not a lack of effort—it may be understanding how the nervous system is communicating.

When Sibling Conflict Is Really a Regulation Problem

Does your child with ADHD or neuro divergent child seem to constantly pick on a sibling?

Before asking, “Why are you being mean?” try asking:

“What is your child’s nervous system trying to tell us?”

Impulsivity, sensory-seeking, boredom, big emotions, and difficulty stopping can all contribute to sibling conflict.

At BalanSense-Therapy, LLC, we look beyond the behavior. Through our Polyvagal-informed, Regulation First approach, we help families understand the connection between:

🧠 Nervous system regulation
🤝 Co-regulation
👋 Sensory needs
💛 Emotional regulation
🎯 Executive functioning
🏡 The home environment

But there is another important piece:

Parents need regulation skills, too.

Children learn regulation through repeated experiences of co-regulation with safe, regulated adults. When parents learn to recognize their own nervous-system responses, regulate themselves, and respond rather than react, they can create a different environment for their children to learn these same skills.

For some families, parent self-regulation and co-regulation coaching may be an important part of the process. When a parent is also carrying the effects of significant stress or trauma, appropriate trauma-informed support, including SSP (Safe and Sound Protocol) when clinically appropriate, may help support the parent's own regulation journey.

This isn't about blaming parents.

It's about supporting the whole nervous system of the family.

Instead of shame:
❌ “What's wrong with you?”

Try:
❤️ “Your body is having a hard time. Let's help it slow down, and then we'll work on what happened.”

Regulation doesn't mean there are no boundaries.
It means helping children become regulated enough to learn, connect, problem-solve, and practice appropriate boundaries.

The goal isn't perfect siblings.

It's fewer conflicts, faster recovery, safer interactions, stronger relationships—and children learning skills they can carry into adulthood.

💬 Parents: Have you noticed that when YOU are overwhelmed, your child's reactions become harder to manage, too?

This is where parent regulation + child regulation + co-regulation can become an important part of the picture.

When Your Child Is Melting Down… What If Their Body Is Saying, “Something Is Wrong”?

Have you ever watched your child suddenly:
• Have a meltdown over something that seems small?
• Stick their tongue out, yell, or become verbally reactive?
• Hit, kick, push, or physically attack a sibling or parent?
• Say, “I don't know why I'm mad”?
• Complain of a stomachache when they are frustrated, angry, or overwhelmed?
• Feel nauseous or say, “I feel sick”?
• Develop a headache or migraine?
• Become dizzy or lightheaded?
• Experience blurred or distorted vision?
• Seem unable to explain what they are feeling?
As parents, these moments can be incredibly difficult. We may immediately think, “Why are you behaving this way?” or “You know better.”
But sometimes the more important question is:
“What is happening inside your child's body right now?”
When a child's nervous system becomes overwhelmed, their ability to process emotions, sensory information, and internal body sensations can change. A child may experience intense stomach sensations, nausea, headaches, dizziness, or changes in how they perceive their surroundings without understanding what is happening or having the words to explain it.
They may simply know:
“I feel bad.”
“I feel angry.”
“I need to get away.”
“I don't know why!”
The behavior we see on the outside may be an expression of what the child is experiencing on the inside.
This is where interoception—our ability to sense and understand what is happening inside our bodies—becomes important.
Instead of immediately correcting the behavior, we can first help the child regulate:
💙 Connect before correcting.
💙 Use a calm, predictable voice.
💙 Help them identify what they are feeling in their body.
💙 Offer movement or appropriate heavy-work activities when needed.
💙 Give words to sensations: “Your stomach feels tight. Your head hurts. Your body looks overwhelmed.”
💙 Teach them that emotions and body sensations are signals—not something to be ashamed of.
At BalanSense-Therapy, LLC, our “Safety First” Polyvagal-informed Occupational Therapy approach looks beyond the behavior. We help children and parents understand the connection between the nervous system, sensory processing, interoception, emotions, and behavior.
Through co-regulation, sensory strategies, the Safe and Sound Protocol (SSP), Focus iLS, and individualized sensory support, we help families develop practical tools to move from overwhelm toward greater regulation, understanding, and confidence.
Your child's behavior is communicating something.
Let's learn to listen to what their body is trying to say. 💙
