Two weeks after suffering a heart attack, 37-year-old Maria Simpson received another shock: a notice from Cigna denying coverage for services it considered “not medically necessary.”
Simpson said in a widely viewed TikTok that charges for her two-day hospitalization had already reached $110,000. Her care included an emergency-room visit and treatment in a cardiac catheterization lab.
“They don’t think that saving my life was medically necessary,” Simpson said.
Cigna says emergency services do not require advance authorization, although hospital admissions following emergencies must be reported and can undergo review. Federal rules generally require insurers to judge emergency claims according to the symptoms that prompted the visit, rather than solely by the final diagnosis, according to federal guidance.
Simpson can challenge the decision through an internal appeal and, if necessary, an independent external review, HealthCare.gov says. But the denial leaves a heart-attack survivor navigating a second emergency: determining whether lifesaving care will become life-altering debt.
