Xét nghiệm Lipoprotein (a) và Apolipoprotein B là gì?
Vì sao ngày càng được quan tâm trong tầm soát tim mạch?

Lipoprotein (a) – Lp(a)
Lipoprotein (a) là một dạng cholesterol đặc biệt có cấu trúc tương tự LDL-cholesterol (“cholesterol xấu”), nhưng gắn thêm một loại protein gọi là apolipoprotein(a). Chính cấu trúc này khiến Lp(a) dễ thúc đẩy quá trình hình thành mảng xơ vữa, gây viêm và tăng nguy cơ hình thành cục máu đông trong động mạch.
Điều đáng lưu ý là nồng độ Lp(a) chủ yếu do di truyền quyết định và thường không giảm nhiều nhờ chế độ ăn uống hay các thuốc hạ cholesterol thông thường. Người có Lp(a) cao có thể đối mặt với nguy cơ bệnh tim mạch, đột quỵ hoặc nhồi máu cơ tim cao hơn dù các chỉ số cholesterol khác vẫn ở mức bình thường.

Apolipoprotein B (ApoB)
Apolipoprotein B là loại protein hiện diện trên bề mặt các hạt mỡ có khả năng gây xơ vữa động mạch như LDL, VLDL và IDL. Mỗi hạt mỡ “xấu” này đều chứa một phân tử ApoB, vì vậy xét nghiệm ApoB giúp đánh giá chính xác số lượng các hạt mỡ gây hại đang lưu thông trong máu.
Khác với xét nghiệm mỡ máu truyền thống chỉ đo tổng lượng cholesterol, ApoB phản ánh trực tiếp “số lượng hạt mỡ gây xơ vữa”. ApoB càng cao thì nguy cơ tắc nghẽn mạch máu và bệnh tim mạch càng lớn.

Các xét nghiệm Lipoprotein (a) và Apolipoprotein B thường được cân nhắc trong những trường hợp:
• Có tiền sử gia đình mắc bệnh tim mạch sớm
• Cholesterol cao khó kiểm soát
• Đã có yếu tố nguy cơ tim mạch như tăng huyết áp, tiểu đường, hút thuốc lá
• Cần đánh giá nguy cơ tim mạch chuyên sâu hơn

Trong một số trường hợp, cholesterol tổng thể hoặc LDL-C có thể chưa quá cao nhưng ApoB hoặc Lp(a) lại tăng đáng kể — đây có thể là dấu hiệu cảnh báo nguy cơ tim mạch tiềm ẩn mà xét nghiệm thông thường chưa phản ánh đầy đủ.

Heart disease isn't always caused by lifestyle alone. For some people, genetics play a significant role.

Lipoprotein(a), or Lp(a), is a type of cholesterol-carrying particle that is largely determined by your genes. While everyone has some Lp(a), approximately 1 in 5 people inherit elevated levels, increasing their lifetime risk of heart disease, stroke, and aortic valve disease.

Because Lp(a) is inherited and not routinely included in a standard cholesterol test, many people are unaware they have an elevated level until a cardiovascular event occurs.

𝗪𝗵𝘆 𝘀𝗵𝗼𝘂𝗹𝗱 𝘆𝗼𝘂 𝗰𝗼𝗻𝘀𝗶𝗱𝗲𝗿 𝘁𝗲𝘀𝘁𝗶𝗻𝗴 𝘆𝗼𝘂𝗿 𝗟𝗽(𝗮)?

1. It helps identify inherited cardiovascular risk.
High Lp(a) is one of the strongest inherited risk factors for cardiovascular disease. If you have a family history of heart disease or stroke—particularly at a younger age—testing may provide valuable insight into your long-term risk.

2. One test is usually enough.
Unlike many other health markers, Lp(a) levels are largely established in early childhood and remain relatively stable throughout life. In most cases, a single measurement provides a reliable assessment of your inherited risk.

3. It isn't included in routine cholesterol screening.
A standard lipid profile measures total cholesterol, LDL, HDL, and triglycerides—but Lp(a) is not routinely tested. This means you may have normal cholesterol levels while still carrying an increased inherited cardiovascular risk.

4. Healthy habits are still important—but they may not lower Lp(a).
Regular exercise, a balanced diet, and maintaining a healthy weight are essential for heart health, but they have little effect on inherited Lp(a) levels. Researchers are currently studying new treatments specifically designed to reduce Lp(a).

5. Knowing your risk helps guide prevention.
If your Lp(a) level is elevated, your doctor can work with you to optimise other modifiable cardiovascular risk factors, including LDL cholesterol, blood pressure, blood sugar, inflammation (such as hs-CRP), weight, and overall metabolic health. Early identification allows for a more personalised approach to reducing long-term cardiovascular risk.
