"Doctor, what's the difference between palliative care and hospice care?"

I get this question asked a lot of times. Not just by my patients. Often times their family members, my own colleagues in different fields, my own family members and friends. I get asked this question almost every time during radio, TV and newspaper interviews.

It does not get easier with a lot of articles (usually originating from specific countries) entitled palliative care vs hospice care.

I'll explain to you about what these questions mean in our Malaysian context. Many countries also follow the same concept.

Let's get the first thing right. If the word 'versus' is part of your question, you're asking the wrong question. That's why this concept is very difficult to grasp.

The right way to explain it is how can this two work together for you?
palliative care is a branch of medicine where the approach is to improve your quality of life when you are diagnosed with a life-threatening illness. It helps you and your family cope with the multidimensional issues related to care, be it physical, psychosocial and spiritual. We help to address these issues after a careful assessment.

Our work prioritises comfort. We help to alleviate symptoms - whatever it may be. We also help you and your family make sense of the changing reality when facing serious illnesses.

Palliative care can be provided by a multidisciplinary team in a hospital that helps to address many different issues affecting a person affected by serious illness. Often, patients' ability to function reduces and they become unable to come to the hospital very often.

Remember that hospitals are not built for long-term care. Even if we do, most patients do not feel hospital stay aligns with their wishes when it comes to care.

Our hospices work by having a multidisciplinary community team. These organisations help to deliver palliative care in the community by arranging regular, scheduled visits.

Where an area is not readily serviced by an NGO Hospice, Ministry of Health domiciliary programs have begun to step up to provide this care too.

Here are some important things to remember:
1️⃣ You don't have to choose one or the other. Most of the time, the hospital and hospice team will try to work together to align care with your wishes.

2️⃣ Receiving these teams to deliver care does not come with a predicted survival time. No one has to decide your expected survival is within a certain period of time. The referring doctor just need to be clear that your disease is life-threatening and curative options are difficult/limited.

3️⃣ If you need to be hospitalised for appropriate reasons/interventions, this can still be arranged. You can even still be on chemotherapy or other treatment.

and are not about giving up hope. It's about aligning those hopes with your wishes and the reality of your condition. It's about caring for a person whilst maintaining dignity and comfort.

🏠 From Humble Beginnings in 1994 to 1,871 Visits in 2025: Help Hospis Melaka Sustain Our Growing Compassion! 💙

Since Hospis Melaka was founded in 1994, our core mission has never wavered: to provide 'Care and Comfort' to patients facing advanced cancers, completely free of charge.

While we are proud of our history, the landscape has changed. The need for palliative home care in our community has grown exponentially.

Look at the impact we have achieved together since 2011:
In 2011, we managed 164 home visits and 71 patient referrals.

In 2025, we project 1,871 home visits and 305 patient referrals—a massive increase in patients requiring our complex care, equipment loans, and emotional support.

This chart shows a triumph of community care, but it also shows a tremendous increase in costs. Every visit represents a nurse’s travel, vital medical supplies, and oxygen or hospital bed provision.

We must continue to meet this demand. We need your help to keep our nurses on the road and our equipment available.
