For over 33 years we have partnered with families caring for an aging parent.

Aging Well Rochester, a Division of Reflections Management & Care, provides compassionate care management, advocacy, dementia support, PRI/screens, and in-home care services helping older adults age with dignity and support. Our goal is to listen to your concerns and use our professional skills and knowledge of community resources to create a custom plan for care and support that meets your parent

s' needs. Each member of our Care Management Team is a licensed Masters level social worker and/or registered nurse. We have expertise working with medically complex older adults and those suffering from memory loss.

* In-home assessment
* Expertise on Alzheimer Disease
* Coordinate and supervise in-home services
* Counseling
* Placement in senior residences, assisted living and nursing homes
* Information on Medicare, Medicaid & Long Term Care Insurance
* Liaison with families at a distance, overseeing care

As a private practice not affiliated with any service provider, we can make candid and confidential recommendations based on what is best for your parent. Today, we pause to honor and remember the brave men and women who gave their lives in service to our country. Memorial Day is more than a long weekend — it is a time of reflection, gratitude, and remembrance for the sacrifices made to protect the freedoms we hold dear.

At Aging Well Rochester, we hold close the memory of those who served with courage and selflessness, and we extend our deepest respect to the families and loved ones who carry their legacy forward every day.
