What You Need to Know About Unexplained Weight Gain and the Risks of Diabetes
I. Possible Mechanisms Behind Unexplained Weight Gain
Insulin Resistance
When body cells become less sensitive to insulin, insulin levels rise to keep blood sugar stable. High insulin levels promote fat storage and inhibit fat breakdown, which can lead to long-term weight gain.
Hormonal Imbalances
An underactive thyroid, elevated cortisol levels caused by chronic stress, and fluctuations in oestrogen or progesterone can slow metabolism and encourage fat accumulation.
Medication Side Effects
In South Africa, certain antidepressants, corticosteroids, hormonal contraceptives, and insulin-based treatments are also linked to weight gain.
Lack of Sleep and Stress
Insufficient sleep and long-term psychological stress increase ghrelin levels (the โ€œhunger hormoneโ€) and reduce leptin levels (the โ€œsatiety hormoneโ€), leading to increased appetite and weight gain.
๐Ÿฉ II. The Dangers of Diabetes (Especially Type 2 Diabetes)
Significantly Increased Risk of Cardiovascular Disease
People with diabetes have a two- to four-fold higher risk of heart attacks, strokes, and high blood pressure. This link is also well documented in South Africa.
Kidney Damage (Diabetic Nephropathy)
Chronically high blood sugar levels damage the kidney filters (glomeruli), leading to proteinuria and, in advanced stages, kidney failure.
Neuropathy and Limb Complications
Persistent high blood sugar can damage peripheral nerves, causing numbness, tingling, and foot ulcers. In severe cases, amputation may be necessary.
Retinopathy and Risk of Blindness
Diabetes can damage the small blood vessels in the retina, leading to visual impairment, macular oedema, and in extreme cases, blindness.
Infections and Weakened Immune System
Long-term high blood sugar levels promote bacterial and fungal infections and increase the risk of skin and urinary tract infections.
Metabolic Syndrome
Weight gain + insulin resistance + lipid disorders + high blood pressure = a significantly increased risk of heart and cerebrovascular diseases.

More and more people are becoming overweight. Since 1975, the number of overweight people worldwide has tripled. In 2016, around 1.8 billion adults were considered overweight, including about 650 million who were severely overweight. In medical terms, this condition is also known as obesity.
In South Africa, being overweight is also a widespread health problem. About half of the adult population is overweight, with a noticeably higher proportion among men than women. Around one in six to seven adults is considered obese, and the number continues to rise. Particularly worrying is that more and more children are affected: a significant proportion of school-aged children in South Africa are already overweight.
