Pain after surgery...Patients often dont get told this important information before surgery (it doesnt just apply to spinal surgery). Physiotherapy can help with many post op issues. We've recently seen several patients post spinal surgery with ongoing pain. A combination of sofrt tissue release, stimpod to get the nerces firing properly, education and exercise can address the issues discussed in this post. Some of these patients suffered for months and in one case years before being referred to physio ๐Ÿ˜” and now they are finally getting back to doing the things they love โค๏ธ ๐Ÿ˜

Why are you still in pain after spine surgeryโ€ฆ even when everything went perfectly?

This is one of the most honest conversations I have with patients.

The surgery may have successfully decompressed the nerve, corrected the instability, or removed the herniation. The imaging looks clean. The hardware is in perfect position. And yet the pain is still there.

Hereโ€™s why that can happen:

โ€ข Nerves that were compressed for months or years donโ€™t always โ€œswitch offโ€ the moment pressure is gone. They can stay irritated or hypersensitive for a long time.

โ€ข Scar tissue (epidural fibrosis) forms as part of normal healing and can tether or irritate a nerve root.

โ€ข The original pain generator wasnโ€™t the only one โ€” facet joints, sacroiliac joints, or adjacent levels can become the new source once the main problem is fixed.

โ€ข Your deep stabilizing muscles (especially the multifidus) can stay inhibited โ€” essentially โ€œturned downโ€ by the nervous system โ€” even after the structural problem is fixed. This is a protective response that often lingers and makes full recovery harder.

โ€ข In some cases the nervous system itself has become sensitized โ€” a real biological change, not โ€œin your head.โ€
Successful surgery does not always equal immediate zero pain. Healing is rarely linear.

What we typically evaluate next:

โ€ข A detailed clinical re-assessment and careful review of your imaging โ€ข Diagnostic injections or blocks to pinpoint the true pain generator โ€ข Specialized physical therapy focused on reactivating the deep stabilizers and restoring motor control

If youโ€™re months out and still struggling, it doesnโ€™t automatically mean the surgery failed. It means you need a careful look at why the pain is persisting.

Its never too late to get stronger! ๐Ÿ’ชAs we age we naturally lose muscle strength which is why its important to stay active and purposefully do things which maintain our strength, balance and proprioception.๐Ÿ’ƒ Simple leg extensions made a massive difference to these 90 year old, imagine how much you could do by committing to one exercise a day. Ask your physio where to start. It doesn't have to be a half hour run or a gym session. Start with 10 squats everytime you use the bathroom, 10 bicep curls everytime you boil the kettle or stand on 1 leg when you brush your teeth. Let us know how it goes๐Ÿ˜€

Yes! Those of you that have done back rehab with me will know about this already. ๐Ÿ™ƒThe bonus is, most of these exercises when done correctly aren't painful on your back (talk to your physio if they are) with the exception of the deadlift which many people find not only painful but also fear inducing. When rehabilitation your back, its imperative that you feel safe (and not thinking you will reinjure...) In my experience, the deadlift needs some preparation and modifications in many patients before going into the full exercise. This will build confidence & mobility. Talk to your physio to guide you ๐Ÿ‘Œ๐Ÿ’ช
