Not all substance use means that someone has an addiction. However, both prescribed and recreational use can become concerning.

Medicinal use:

Some medications are used to manage pain, sleep difficulties, anxiety and other health conditions. When taken as prescribed and regularly reviewed by a healthcare professional, they have a clear medical purpose.

Concern may arise when someone takes more than prescribed, uses another person’s medication, combines it with other substances or feels unable to manage without it.

If you are worried about prescribed medication, encourage your loved one to speak honestly with the prescribing doctor. They should not suddenly stop taking medication without medical guidance.

Recreational use:

Recreational use refers to alcohol or other substances used socially or for pleasure. It does not automatically mean that someone has an addiction.

It may be becoming problematic when the person begins using more often, taking greater risks, hiding their use or experiencing consequences in their health, relationships, responsibilities or finances.

Addiction:

Addiction involves more than how often someone uses a substance. The person may struggle to control their use, give it increasing priority and continue despite the harm it is causing.

The most helpful questions are often:
• Can they control or reduce their use?
• Is it affecting their health or daily life?
• Is it damaging relationships or trust?
• Are they continuing despite repeated consequences?
• Does using feel more important than other responsibilities or activities?

Families do not need to decide which label fits. If you have noticed a concerning pattern, a professional assessment can help you understand what is happening and what kind of support may be appropriate.

When someone you love is struggling with addiction, conversations can quickly become emotional.

You may be frightened, angry, exhausted or deeply hurt. In those moments, it is understandable that words come out more sharply than you intended.

Supportive language does not mean minimising what has happened, hiding your feelings or avoiding difficult truths. You can speak with compassion while remaining honest about the impact the addiction is having on you and your family.

It can help to:
• describe what you have noticed
• explain how it is affecting you and the family
• speak clearly about what needs to change
• avoid shame, labels and personal attacks
• focus on support, treatment and the next steps

There is no perfect phrase that will make someone accept help. They may still become defensive, deny the problem or refuse to talk. Your words cannot control their response.

The aim is to communicate with care and clarity while remaining honest about the harm caused and the support your family needs.

It is not always easy to recognise when someone’s substance use has become a problem.

As a parent or partner, you may notice changes but question whether you are overreacting. You may also hear promises that it is under control or that it will not happen again.

Often, it is not one isolated incident but a growing pattern that signals the need for support. Signs may include:
• using more often or in larger amounts
• trying to cut down but being unable to
• needing substances to relax, cope or function
• becoming secretive or dishonest about their use
• neglecting responsibilities at home, work or school
• losing interest in relationships or activities they once valued
• changes in sleep, mood, appearance or personal care
• increasing financial difficulties
• taking risks while using, such as driving
• continuing to use despite the effect on their health, relationships or daily life

One sign alone does not necessarily mean that someone has an addiction. What matters is the pattern, whether the changes are increasing and how much the substance use is affecting their life and the people around them.

You do not need to wait for a crisis or “rock bottom” before seeking guidance. A professional assessment can help you understand what is happening and what kind of support may be appropriate.

Even if your loved one is not ready to seek help, you can still speak to a professional about your concerns and how best to respond.
