💙 Happy International Youth Day!

Today, I’m celebrating the young people who bring so much energy, creativity and possibility into our world. Every young person deserves the space to dream, grow, find their voice and become everything they are capable of becoming.

And as a paediatric pulmonologist, I’m reminded every day that **good health is part of that journey**. Looking after your lungs, protecting them from to***co smoke and va**ng, staying active and speaking up when something doesn’t feel right are small choices that can make a big difference over time.

To every young person out there: **keep breathing, keep dreaming and keep becoming.** Your future is worth protecting. 💙🌱

When we think about lung cancer, we often think of adults. However, healthy lungs are built long before adulthood begins.

A child's lungs continue to grow and develop throughout childhood and adolescence, making these early years a critical window for lifelong respiratory health. Protecting developing lungs today can have lasting benefits for the future.

Simple steps can make a meaningful difference:
🌿 Avoid exposure to to***co smoke and va**ng.
🩺 Seek early treatment for persistent respiratory symptoms.
💙 Encourage clean indoor air and healthy environments for growing lungs.

While lung cancer is extremely uncommon in children, nurturing healthy lungs from an early age helps lay the foundation for stronger respiratory health throughout life.
This World Lung Cancer Day, let's work together to protect every child's right to breathe, grow, and thrive.

🚨 Post-Submersion Monitoring: When to Seek Immediate Medical Evaluation.

A dangerous phenomenon can occur hours after a child has accidentally swallowed or inhaled water during a near-drowning incident. I urge all parents to closely monitor their child for at least 24 to 48 hours following any water incident, no matter how brief or minor it initially seemed. Immediate medical assessment at an emergency facility is strictly required if you notice any clinical warning signs, including a persistent or worsening cough, an unnaturally fast breathing rate, unusual lethargy or extreme fatigue, dropping oxygen saturation levels, or visible signs of respiratory distress like chest retractions or flared nostrils.

Never wait for symptoms to worsen when dealing with potential lung injury; proactive medical evaluation is lifesaving.

🫁 The Clinical Reality: What Happens to a Child’s Lungs After Drowning? Even when a child is successfully rescued from a submersion incident, the medical journey does not automatically end at the poolside.

When a child submerges, the aspiration of even a microscopic amount of water into the respiratory tract disrupts the vital pulmonary surfactant (the specialized fluid coating that keeps the air sacs open). This leads to severe acute lung injury, intense localized inflammation, and fluid accumulation, which can rapidly progress into Acute Respiratory Distress Syndrome (ARDS) or severe secondary pneumonia. These complex, deep-tissue respiratory complications can compromise oxygen exchange hours after the event.

Understanding that even brief submersion can carry profound, delayed consequences is vital to keeping small lungs safe.

🚫 Debunking the Myths: Drowning Happens Fast and Quietly.

As we approach World Drowning Prevention Day, we must address a dangerous misconception popularized by television: that a drowning child will splash violently and scream for help.

In reality, childhood drowning is almost always a silent, terrifyingly rapid event. When a child is suffocating, their body’s natural instinct focuses entirely on trying to gasp for air, leaving them completely unable to call out or wave their arms. High-risk situations can unfold in seconds around backyard pools, filled bathtubs, or open water basins. True drowning prevention must begin with a foundational shift in parental awareness. Vigilance is our first line of defense.

🥦 Fueling the Fight: How Nutrition Impacts Childhood Lung Health.

We are proud to partner with Outreach to shed light on the inseparable bond between optimal nutrition and sound respiratory function. For a child battling chronic lung disease, breathing requires significantly more physical energy, drastically escalating their daily calorie and protein requirements to sustain growth and repair tissue. Malnutrition creates a dangerous vulnerability, weakening the immune system and drastically increasing both the frequency and severity of respiratory infections. Healthy, resilient lungs fundamentally require balanced nutritional support to thrive.

Every single child deserves the baseline opportunity to grow, build strong immunity, and breathe well.

🤝 Partnering for a Healthier Tomorrow: Healthy Lungs for Life. In a proud collaboration with the South African Thoracic Society , we are highlighting a vital clinical truth: many chronic adult respiratory conditions trace their roots directly back to early childhood.

When conditions like paediatric asthma, bronchiectasis, or recurrent childhood pneumonia are underdiagnosed or poorly managed, they leave lasting structural scars on developing respiratory tissues. Furthermore, early exposure to toxic to***co smoke severely stunts natural lung growth. Protecting young, vulnerable lungs today is the only definitive path to ensuring healthy adult lungs tomorrow. Early intervention changes the entire trajectory of a child's life.

The Hidden Impact of Indoor Air Pollution on Developing Lungs.

We frequently discuss outdoor smog, but the air inside our homes can sometimes be significantly more harmful to a child's developing respiratory tract than the air outside.

In South Africa, many households rely on paraffin heaters, coal stoves, or wood fires for heating and cooking during winter, which release microscopic particulate matter and toxic gases directly into unventilated living spaces. When combined with environmental to***co smoke, these pollutants irritate the delicate linings of a child's airways, predisposing them to chronic lung diseases. In fact, recent environmental studies highlight Johannesburg as one of the most heavily polluted cities, making indoor air quality management even more critical for vulnerable children.

The Truth About Nebulisers: Inhalers versus Nebulisation.

There is a deep-seated myth among many families that nebulisers are inherently stronger, more effective, or superior to standard metered-dose inhalers (puffs) for treating childhood asthma or wheezing. The clinical reality is quite different.

For the vast majority of children, an inhaler used correctly with an appropriate spacer device is just as effective as a nebuliser at delivering life-saving medication directly into the lower lungs, while being significantly faster, highly portable, and causing far fewer side effects like a rapid heart rate or anxiety. Nebulisers are generally only necessary during severe, acute respiratory emergencies where a child is physically struggling to breathe or cannot cooperate with a spacer. The best treatment is always the right treatment, tailored specifically to your child’s clinical state rather than relying on outdated misconceptions.

🇿🇦 Celebrating Nelson Mandela International Day: 67 Minutes for Children’s Lungs.

On Mandela Day, we are reminded that small, collective acts of service can have lifelong positive impacts on a child's health and well-being. This year, we invite our community to dedicate their 67 minutes to protecting vulnerable little lungs. You can make a lasting difference by learning pediatric CPR, helping a family access specialized healthcare services, sharing evidence-based respiratory health information with parents, supporting smoking cessation programs, or volunteering to assist families managing chronic childhood lung diseases. Every action counts toward building a healthier generation.

Let's protect their right to breathe, grow, and thrive.
