Investigating Hypertension in Patients on Antiretroviral Therapy (ARVs): A Call for Scientific Vigilance by SAMLU

Introduction

The Human Immunodeficiency Virus (HIV) epidemic remains one of South Africa’s most pressing public health challenges. The widespread rollout of Antiretroviral Therapy (ARVs) has significantly reduced mortality and improved quality of life for millions of patients.

However, emerging patient complaints and clinical observations suggest a concerning trend: a growing number of individuals on newer ARV regimens are reporting elevated blood pressure and related complications.

The South African Medical Laboratory Union (SAMLU), through its Research Committee, has observed claims that up to 80% of some patient groups report symptoms consistent with hypertension after initiating newer ARV treatments. While such figures require rigorous validation, the concern warrants urgent, structured scientific investigation.

Understanding the Concern: ARVs and Hypertension
Hypertension is a major risk factor for cardiovascular disease, stroke, and kidney failure. It is particularly dangerous in patients already immunocompromised by HIV.

Some possible links between ARVs and hypertension include:
Metabolic Effects: Certain ARVs, especially integrase inhibitors, have been associated with weight gain and metabolic changes.
Endothelial Dysfunction: ARVs may affect blood vessel function, contributing to increased vascular resistance.
Chronic Inflammation: HIV itself, even when controlled, can contribute to long-term inflammation that raises cardiovascular risk.
Drug Interactions: Combining ARVs with other medications may indirectly influence blood pressure regulation.

It is important to note that current global scientific consensus does not support the idea that ARVs are intentionally engineered to cause other complications but currently patients are complaining of hypertension disorders.

We call on all Medical Laboratory professionals to assist in collecting all data and researching on this matter working closely with companies approving such drugs like SAHPRA to corporate with the Scientific Community Professionals concerned about HIV patients.

Laboratory Professionals are the backbone of healthcare diagnostics, yet we remain among the most undervalued, overworked, and silenced workers in the health system.
We generate results that save lives, guide treatment, and shape national health policy — but decisions about our labour, safety, and careers are often made without us.

SAMLU exists because this injustice can no longer be normalised.
