The truth is you never know what you get in a locum/permanent health professional and most medical units rather count on by word-of-mouth references which are few and far between. Not every opportunity that looks good on paper is the right long-term fit.

Before accepting a position or sessions, it’s important to evaluate more than just the salary offer.

Key questions to ask include:
• What does the workload realistically look like? Patient volumes and administrative expectations significantly affect sustainability.
• What support systems are in place? Strong nursing, reception, and operational support impacts daily workflow.
• How is remuneration structured? Understand whether compensation is fixed, sessional, or billing-based.
• What growth opportunities exist? Consider mentorship, clinical exposure, and long-term development potential.
• Are expectations clearly defined? A well-managed practice should communicate responsibilities transparently.

Career decisions shape both professional growth and quality of life.

Did you know? Crazy Socks 4 Docs is a global initiative aimed at raising awareness around mental health within the medical profession.

Healthcare professionals often work under sustained pressure while continuing to care for others, even during periods of personal stress or burnout.

The campaign encourages:
• Open conversations around mental wellbeing
• Reducing stigma in healthcare environments
• Supporting colleagues who may be struggling
• Recognising the importance of seeking support early

Mental health remains an important part of professional sustainability in medicine.
Sometimes small conversations make a significant difference.

Moving from public healthcare into private practice is more than just a change in workplace, it often requires a shift in how medicine is practised operationally and professionally.

Doctors entering private practice quickly encounter responsibilities that may receive less exposure in the public sector.

Common adjustments include:
• Billing and coding systems: Understanding medical aid billing, ICD-10 coding, and practice workflows becomes essential.
• Patient expectations: Private patients often expect shorter waiting times, detailed communication, and continuity of care.
• Practice operations: Doctors become more involved in scheduling, documentation standards, and administrative coordination.
• Professional independence: Decision-making often becomes more autonomous, particularly in smaller practice environments.

The transition can be highly rewarding, but adapting successfully requires both clinical and operational awareness.

Attention all men! Men’s Health Month is a reminder that preventative healthcare remains just as important as treatment.

Many conditions affecting men, including hypertension, cardiovascular disease, diabetes, and certain cancers, are often detected late due to delayed medical consultation.

Key areas men should prioritise include:
• Routine health screenings
• Blood pressure and glucose monitoring
• Mental health awareness
• Lifestyle and cardiovascular risk management

Early intervention improves long-term outcomes.

Encouraging regular check-ups and open conversations around health remains an important part of preventative care.

One of the more complex moments in clinical practice occurs when a patient refuses treatment.

While doctors are trained to act in a patient’s best interests, competent adults have the legal right to decline medical care.

When this happens, several professional responsibilities become important:

• Ensure the patient is fully informed - Explain the diagnosis, recommended treatment, possible alternatives, and the risks of declining care.
• Confirm decision-making capacity - The patient must be able to understand the information and make an informed choice.
• Document the discussion carefully - Clinical notes should clearly reflect the advice given and the patient’s decision.
• Provide follow-up guidance where appropriate - Advise the patient on warning signs or circumstances that should prompt them to seek care again.

Situations like these highlight how communication and documentation remain essential parts of responsible medical practice. More information here:https://www.hpcsa-blogs.co.za/wp-content/uploads/2025/11/ETHICAL-GUIDELINES-TREATMENT-WITHHOLDING-WITHDRAWAL.pdf
