The Posterior Cruciate Ligament — the PCL — is actually the strongest ligament in the knee. Yet it remains one of the most overlooked knee injuries.

PCL tears can happen during a dashboard injury, a fall onto a bent knee, or a tackle from the front. The knee can still feel relatively stable right after the injury, which is exactly why these tears are so often missed.

Most isolated PCL tears don’t require surgery. With the right diagnosis and a structured rehabilitation programme, many patients recover well without going to theatre.

Women are up to four times more likely to injure the ACL in the knee than men.

ACL injuries commonly happen during sport when the knee is placed under abnormal force — often while the knee is slightly bent, changing direction, pivoting, landing, or during contact.

These injuries are common in sports such as netball, soccer, rugby, hockey, and other high-demand activities.

Dr Philip Mostert explains how ACL injuries can also affect other structures in the knee, including the meniscus, cartilage, and medial collateral ligament (MCL).

Symptoms may include rapid swelling within the first 12 hours, instability, pain, difficulty continuing activity, or the feeling that the knee may give way.
