IMPORTANT NOTICE – POLICY REGISTRATION WITH ASITHWALISANE FUNERAL
To successfully register for a policy, the following must be submitted:

* A clear copy of the ID, Birth Certificate, or Passport
* A completed and signed joining form and contract
* The main member’s fingerprint on the joining form
* All documents must be signed by both the main member and an ASITHWALISANE Funeral representative
* All documentation must be submitted to the Head Office within seven (7) days of joining

ASITHWALISANE FUNERAL SERVICES
GOLD PLAN TERMS AND CONDITIONS

Please read these terms carefully, as they outline important information regarding your policy with us.

1. Service Provision
Asithwalisane Funeral will render services according to the selected Gold Plan package.
Note: Only one claim is permitted per deceased individual.

2. Eligibility and Waiting Periods
All dependents must be registered under the policy and are subject to a three (3) month waiting period.

Any individual older than three (3) months of age must be added to the policy to qualify for benefits.

New members are subject to a strict 3-month waiting period.

A two (2) year waiting period applies for deaths resulting from su***de.

If death occurs due to an accident during the waiting period, the first premium must have been paid for the claim to be considered.

Transfers from another funeral parlour:

No waiting period if the policy is fully paid up and in good standing (Flat lid coffin benefit).

1-month waiting period applies if transferring from another provider (Casket/ITC benefit).

3-month waiting period applies when claiming cash benefits.

3. Payments and Lapses
Premiums are due by the 1st of every month.

If premiums are not paid for three (3) consecutive months, your policy will automatically lapse after 37 days of non-payment.

Upon lapse, all benefits will be forfeited, and a new waiting period will apply upon rejoining.

4. Funeral Cover for Children
Children under the age of 13 years are covered for funeral and goods only (stillborns included).

Pregnant mothers must register their pregnancy at any Asithwalisane branch. After birth, the child must be registered within 30 days to be covered (cover includes coffin and delivery only).

Children must be registered by 11 months of age to qualify for child benefits.

5. Transportation
The policy includes transportation coverage of up to 600 km.

Any additional distance will be charged at R12 per km.

The main policyholder is entitled to 2 family cars (within 60 km).

Dependents are entitled to a family bus (within 60 km).
