Sometimes the best consultations are the ones where someone says:
“I’m actually feeling okay, but I’ve been meaning to ask about…”

Blood pressure, cholesterol, contraception, menopause/perimenopause, headaches, tiredness, a funny mole, screening tests, "I want to stop smoking", 'my family history is bothering me', "i am stressing about these side effects" — these are all perfectly good reasons to make an appointment.

Prevention is also medicine.

IS EVERY WHEEZE ASTHMA?

In children aged 5 years and younger, wheezing is often triggered by viral infections and does not always mean that the child has persistent asthma.

Treatment depends on the pattern:

• Occasional wheezing only during a cold: a salbutamol pump with a spacer may be sufficient when symptoms occur.

• Frequent wheezing, symptoms between infections, night-time coughing or severe attacks: the doctor may prescribe a low-dose inhaled corticosteroid controller for 2–3 months and then reassess.

• If symptoms remain uncontrolled, the treatment may need to be adjusted or the child referred to a paed.

An asthma pump used with a spacer is usually just as effective as a nebulizer. Younger children can use a face mask attached to the spacer, while older children can use the mouthpiece.

Montelukast is not usually as effective as an inhaled corticosteroid and may cause behavioural or mood-related side effects in some children.

Please have recurrent wheezing assessed by your doctor, and do not change or stop your child’s asthma medication without medical advice.

New research shows the best treatment for asthma is always a combination inhaler, with salbutamol AND a steroid. So if you have only a "blue" asthma pump (Asthavent, Venteze, etc), this is not the best short term or long term option for you and may even cause harm. Chat to your provider about using a combination pump for a reliver, that also acts as the controller.

Sometimes the most revolutionary thing you can do for your body is REST.

Today I had a conversation with someone along the lines of "I need to get better, because I can't rest." and I responded by saying that even if we give you the best treatment in the world, you need to REST as well. 🫶

If you are a female patient on GEMS or Discovery and you are due for a papsmear, and you do not want the discomfort of a speculum, but you want to know whether you have HPV, which is a precursor of cervical cancer, make an appointment to discuss the possibility of doing an HPV self test to screen for the precursors of cervical cancer, which can be done at home, as your medical aid pays fully for these tests.

Remember that 6 monthly visits for your chronic condition changes the trajectory of your life. We struggle on a daily basis to convince patients to come in for a physical check-up, including blood and urine tests in certain circumstances, and then they simply want their scripts to be renewed. Medical treatment and protocols change every 4 months, and therefore, if you get regular check-ups, you are healthier long term. I have seen this time and time again in my practice.

WHAT DOES YOUR HGT READING MEAN?

An HGT is a finger-prick blood-glucose test. The meaning depends on whether you were fasting or had recently eaten.

FASTING HGT
This means no food or caloric drinks for at least 8 hours.

• Below 5.6 mmol/L: Usually within the normal range
• 5.6–6.9 mmol/L: Higher than normal — arrange further testing
• 7.0 mmol/L or higher: In the diabetes range — this needs confirmation with a laboratory blood test

RANDOM HGT
This can be checked at any time, whether or not you have eaten.

• The result varies according to what and when you last ate
• 11.1 mmol/L or higher, especially with symptoms such as excessive thirst, frequent urination, tiredness or unexplained weight loss, may indicate diabetes and needs prompt medical assessment

Below 4.0 mmol/L is considered low blood glucose, particularly if you feel shaky, sweaty, weak, confused or dizzy.

Important: One finger-prick reading cannot usually diagnose or exclude diabetes.
Your doctor may confirm it with fasting laboratory glucose, HbA1c or another appropriate test.
Pregnancy has different cut-off values.
