TIBIAL BONE STRESS INJURY πββοΈ π¦΄
When it comes to bone stress injuries...tibial bone stress injuries are the most common, they account for 50% of BSI in males and 20% in females.
Why do they occur?
It is due to a mismatch between work load and loading capacity of the bone. Bone breakdown exceeds bone repair. This leads to microfractures within the trabecular bone.
The pain initially presents after a run, but then as the injury progresses, the pain is felt during the run and then progresses to occurring overnight, when at it's most severe.
How do we test for it?

οΈ Pain with single leg hopping is 100% sensitive for BSI.
β
οΈ Less than 10cm of palpable pain along the bone (except in the case of anterior shin pain, expect a stress reaction even with more than 10cm pain as there is a high risk of non union).

οΈ MRI to confirm diagnosis but to also exclude red flags.
Treatment
β
οΈ Full rest from running or lower limb loading activities (this isn't required for MTSS).
β
οΈ The use of a boot or crutches for protected weight bearing and mobilisation.
β
οΈ Non weight bearing training like swimming and cycling
β
οΈ Surgical fixation of anterior cortex stress fractures
Prognosis
β
οΈ 6-8 weeks for posteriomedial stress fractures
β
οΈ 4-6 months for anterior cortex stress fractures
