Looking for nursing care at home but unsure where to begin?

DVA Nursing and CHCS Community Nursing can both support people with clinical and personal care needs at home. However, eligibility, funding and access requirements may differ.

DVA Nursing is available to eligible veteran clients with an assessed clinical need. Support includes medication assistance, wound care, hygiene and personal care, chronic condition support and palliative care. Access generally requires a written referral from an authorised healthcare professional.

CHCS Community Nursing provides personalised nursing care delivered according to each individual’s needs. Services include diabetes management, complex wound care, respiratory support, supra p***c catheter SPC change and continence care, medication support, health monitoring and overnight nursing care.
