A clear MRI doesn’t always mean a stable knee. 🙅‍♂️📸

An ACL graft can fail mechanically without looking torn or dramatic on imaging, meaning scans rarely tell the full story. Instead, a revision conversation is shaped by how the knee actually performs in the real world.

Ongoing pain or swelling often signals that the graft has stretched out, losing its structural tension. This leads to functional instability, where the knee shifts or gives way during routine movements. For an athlete whose lifestyle demands safe cutting, pivoting, and accelerating, even minor mechanical laxity makes returning to sport impossible. 💥🦵🏻

A graft that looks intact on a screen can still fail a patient on the field. Clinical examination, functional testing, and an athlete's sporting goals are what truly dictate whether a revision is needed.

Historically, return-to-play decisions were based on the calendar alone (e.g., "9 months post-op"). Today, modern sports medicine demands a more rigorous, data-driven approach.

Objective performance testing removes the guesswork from postoperative progression by measuring precise metrics:
🦵🏻 Is the surgical leg safely matching the strength of the healthy leg?
💥 Can the joint handle explosive power and deceleration?
⚙️ How does the knee control dynamic cutting and jumping?

This data doesn't replace the experience of your surgeon or physiotherapist - it unites them. It gives your entire care team a clear report card of exactly where your recovery stands. When you step back onto the field, you shouldn't have to hope your knee holds up. You should have the data to prove it. 💙

Every knee preservation case is unique, but the most compelling ones share a common feature: a young or active patient with a focal, localised problem rather than widespread joint wear. 🧬🔬

When we talk about knee preservation, we are looking at joint-saving strategies like:
🦵🏻 Limb Realignment (Osteotomy): Offloading a damaged section of bone by subtly adjusting the leg's mechanics.
🦴 Meniscal & Osteochondral Allograft Transplants: Biologically replacing severely damaged cartilage or meniscus using donor tissue.

Why are these cases worth discussing? Because they bridge the gap for patients who don’t fit the criteria for standard joint replacements but cannot function with their current pain levels.

It requires meticulous planning. 📋 We have to analyse the limb’s overall tracking, the specific depth of the cartilage lesion, and the patient's individual sporting demands. When those alignment and pathology features line up perfectly, a biological preservation strategy can truly change the trajectory of a patient's physical future.

Alignment, load distribution, meniscal integrity, stability and joint function all shape how cartilage pathology develops and how it should be managed. Looking at the lesion alone can miss the broader mechanical picture. 🧩

That is why cartilage assessment needs to consider the knee as a system, using clinical reasoning to guide planning, procedure selection and long-term joint preservation.

When is a knee actually ready to return to play after an ACL reconstruction? Historically, surgeons looked at the calendar. Today, we look at the data. 📊🏃‍♂️

As we mark Exercise Right Week, it’s the perfect time to highlight how sports medicine has shifted away from time-based recovery toward a criteria-based model.

We no longer ask, "Has it been 9 months since surgery?" Instead, we ask, "Does your quad strength, limb symmetry, and movement quality actually match the demands of your sport?" 🧐 Passing this test requires a team effort. It relies heavily on the expertise of Accredited Exercise Physiologists and physiotherapists who guide the grueling rehab process. But to know if that rehab is working, we need objective numbers.

This is exactly why we utilise the POSM Performance Lab.

Rather than guessing if a knee is stable enough to cut or pivot, this testing space allows us to measure force production and movement biomechanics in real-time. This objective data gives the patient, their physio, and myself a clear blueprint of exactly where their recovery stands - taking the anxiety out of the return-to-sport transition.

Time tells you how long someone has been recovering – not whether they’re actually ready. ⏱️

Criteria-based progression is more demanding than ticking off dates on a calendar, but it’s also more honest about what recovery actually involves. When considering readiness to return to sport, we assess factors such as:

🧠 Neuromuscular control
🦵 Strength symmetry
📊 The knee’s ability to absorb and generate force under real conditions
🤝 The patient’s psychological trust in the joint.

Those things don’t arrive on a schedule.

Across complex knee surgery, ACL reconstruction or multi-ligament reconstruction, the timeline is a guide, not a finish line.

Sports medicine innovations in load monitoring, objective strength testing and movement analysis are giving clinicians better tools to make those progression decisions.

The promises are everywhere. The evidence is more complicated. 🧬
- PRP has legitimate but modest symptomatic value in selected patients.*
- Stem cells are promising but not yet clinically ready at scale.**
- True cartilage reversal in humans remains unproven.***

The most evidence-backed ways to slow joint ageing are not glamorous. We’re talking strength training, load management, and maintaining a healthy weight. 🦵

While the biohacking narrative gets ahead of the evidence, stem cell therapy, PRP and regenerative medicine are genuinely exciting areas of research. Some applications are showing real promise. But, at this point, there’s a significant gap between what’s being marketed and what’s been proven. 🔬

Always a pleasure to be involved in some medical education with the team at in Singapore. The brand new Arthrolab only opened this week, amazing facility!
This was a special one for me - it's always a nice surprise to see who I'm chairing these education events with. This time was particularly nice, being joined by - a particularly skilled surgeon, thought leader and all round gentleman. It was an honour! Thanks to all who work hard to put these events together.
